亚麻醉剂量氯胺酮用于早孕流产镇痛的临床效果

摘　　要：目的研究亚麻醉剂量氯胺酮静脉注射用于早孕流产镇痛的临床效果。方法对138例无流产禁忌证的早孕患者实施亚麻醉剂量氯胺酮(0.3～0.5mg/kg)静脉注射,氯胺酮的总药量均值为(16.4±2.5)mg。静脉注射2min后施术。观察术中的镇痛效果,测量手术前后的血压、呼吸频率及宫腔内容物的体积。结果亚麻醉剂量氯胺酮静脉注射后无痛及轻度疼痛占92.1%,中度疼痛占7.9%,强度疼痛及以上为0。用药后血压升高(16.5±3.8/13.5±3.0)mmHg(P<0.01),呼吸频率增快(3±1)次/min(P<0.01),术后宫腔内容物比对照组增加(5.3±1.5)ml(P>0.05)。结论亚麻醉剂量氯胺酮静脉注射用于早孕流产镇痛具有起效迅速、价廉高效、副作用少的特点,值得推广。Objective To study the clinical effects of sub-anesthesia doses drugs of ketamine on induced first-trimester abortion.Methods Sub-anesthesia doses of ketamine,which was0.3-0.5mg /kg,were injected in138patients with the first-trimester pregnancy,without abortion contraindication.And the total average doses of ketamine was(16.4±2.5)mg.The operation was performed two minutes after injecting ketamine,while observing analgesia effects,and measuring blood pressure,breath frequency and volumes of intrauterine conteuts.Results No or mild,middle,severe pain after operation were92.15%,7.9%,0,respectively.On the other hand,the rise of blood pressure were(2.2±0.5)/(1.8±0.9)kPa(t=3.53,P<0.01),the increase of breath frequency was(3±1)beats per minute after injection than before(t=3.59,P<0.01),and intrauterine contents increased(5.3±1.5)ml after operation than before(P>0.05).Conclusion The method,in which sub-anesthesia doses of ketamine are used has rapid effect,inexpensive cost and less side-effect,thus is worthy of extensive use.
