急性心肌梗死患者血浆肾上腺髓质素与内皮素的测定及其临床意义

摘  要:应用放射免疫法测定 4 0例急性心肌梗死 (AMI)和 30例正常对照者的血浆肾上腺髓质素 (ADM)和内皮素 (ET- 1)水平 ;同时 ,用彩色超声心动图测定 4 0例 AMI患者的左室射血分数。结果 :AMI组血浆 ADM水平显著高于正常对照 (P<0 .0 5 ) ;AMI患者按心功能分组 ,各组间血浆 ADM差异有显著性 (P<0 .0 5 )。 AMI组血浆 ADM水平与 ET- 1呈显著正相关 (r=0 .5 74 3,P<0 .0 1) ,左室射血分数呈显著负相关 (r=- 0 .6 15 2 ,P<0 .0 1)。认为 ADM参与了 AMI及心力衰竭的病理生理过程 ,血浆 ADM升高可能与 ET- 1升高有关。To observe the changes of plasma adrenomedullin (ADM) and Endothelin (ET 1) levels in patients with acute myocardial infarction (AMI ) and to study correlation between plasma ADM and ET 1, plasma ADM and ET 1 levels were determined by radioimmunoassay in 40 patients with AMI and 30 healthy control subjects The left ventricular ejection fraction(LVEF)of 40 patients with AMI was measured by echocardiography The results showed the plasma ADM and ET 1 levels in patients with AMI were higher than those of healthy control significantly(P<0 05) There were also significant differences among different classes of cardiac performance according to Killip classification in AMI(P<0 05) There was a significant correlation between plasma levels of ADM and ET 1(r=0 5743,P<0 01) LVEF inversely correlated with plasma ADM levels in the patients with AMI(r= 0 6152,P<0 01) These results suggest that ADM might play a role in the pathophysiology of AMI, the elevated plasma ET 1 may be responsible for the rise of ADM levels in patients with AMI
