126例腰椎间盘突出症手术治疗效果不佳原因分析

摘　　要：目的 探讨手术治疗腰椎间盘突出症疗效不佳原因的预防办法。方法 对72年以来手术治疗2000例腰椎间盘突出症中的126例疗效不佳的病例资料进行了临床分析。结果 手术疗效不佳原因是综合性的,其中诊断错误11例;手术适应症掌握不严10例;手术中操作中不细发生并发症51例;术后感染与血肿压迫5例;术后粘连30例;术后下床活动过早18例。结论 手术治疗腰椎间盘突出症虽是一种有效的常规治疗办法,但仍从诊断、手术指征、术中操作、术后处理等进行严格规范,只有这样才能取到预期的效果。Objective To explore the cause and ment for heniation of lumbar intervertebral disc. the method preventment of inffective surgical trat- Methods from 1972 to 2002,2000 patients withlumbar disc herniation treated by surgical discecto-my were reviewed, 126 cases had poor operative results were analysis. Results the causes of failed surgical discectomy are general: 11 cases with false rliagnos;10 cases with vague operative indication;51 cases with postoperative complicaation following careless intraoperative management;5 cases with postoperative infection and hematomaic compression; 30 cases with postoperative attachment; 18 cases with early active weight-bearing mothion. Conclusion Surgical discectomy for lumbar disc herniation is a effective method of treatment,But improving the result of surgical interference,diagno-sis,operative indication,intraoperative management,as well as postoperative care must be emphasized exactly.
