新生儿缺氧缺血性脑病血渗透间隙与阴离子间隙的研究

摘  要:目的 探讨新生生儿缺氧缺血性脑病(HIE)血液渗透间隙(OG)与阴离子间隙(AG)的特点,方法 测定60例HIE患儿的血清电解质、血糖、尿素氮及血清渗透压,计算AG与OG,据此分析HIE患儿AG与OG的特点。结果60例患儿中,39例AG>16mmol/L,为65%;49例PG>10mosm/kg.H2O,为81.67%,5例OG>43mosm/kg.H2O,为8.3%;8例死亡儿治疗前后血OG无明显差异。结论 HIE患儿以高AG性代谢性酸中毒为特点。同时,由于乳酸等内源性物质浓度增高,血OG同度增高,血OG同步增高。动态观察血AG、OG的变化,能反应病情的转归,对疗效观察及预后判断有一定的参考价值。Objective To investigate the characteristic of blood osmolal paps(OG)and anion gaps(AG) of hypsoxic -ischemic encepalopathy (HIE)in neonates. Methods Determing the serum electrolytes, blood sugar,urea nitrogen and osmotis pressure of 60 neonates with HIE,calculating AG and OG,Analyzing the characteristics of AG and OG in neonates with HIE. Results In 60 neonates with HIE,39 cases'AG > 16mmol/L,be-ing65%;49 case'OG>10mosm/kg.H2O,being 81.67%, 5 cases'OG >43mosm/kg.H2O,being8.3% ;before and after the treatment,the OG of 8 death neonates have not the obvious difference. Conclusion HIE patients were characterized by the high AG metabolic acidosis.At the same time,because the increase degree of material of inside body of lactic acid,etc,The blood OG synchronously increaseses higher.Continuously observating the change of blood AG and OG,can respond lapse to state of an ill-ness,and having Certain referrence value for effect and prognosis estimate.
