摘  要:目的 用动物模型方法研究烧伤合并海水浸泡早期损伤机理和特点。方法 Wistar大鼠 175只 ,雄性 ,体重 2 0 6~ 2 90g。动物模型 :制造 30 %深Ⅱ°烧伤。海水浸泡方法 :水温 2 0℃~ 2 2℃。大鼠缚于固定板上 ,双前肢以下浸泡于海水中。分组 :正常对照组 :n =15 ;单纯烧伤组 :n =5 0 ;烧伤 +浸泡海水 2h组 :n =5 0。烧伤 +浸泡海水 3h组 :n =40 ;烧伤 +浸泡海水 4h组 :n =2 0。观测指标 :死亡率统计、渗透压、血常规、血生化。结果 死亡率 :普通烧伤组为 6 %,烧伤 +浸泡 2h为 10 %,烧伤 +浸泡 3h为 2 5 %,烧伤 +浸泡 4h为 5 0 %;血浆晶体渗透明显升高 ,浸泡时间越长 ,渗透压升高越明显 ;海水浸泡烧伤的血液生化改变较普通烧伤明显 ,以血氯、血糖和酶类升高较大 ;酸中毒严重且持续时间长 ;血管通透性增加持续时间明显延长。结论 ①大鼠烧伤合并海水浸泡死亡率明显高于普通烧伤并随浸泡时间延长而增加。②酸中毒相当严重且持续时间长。③血液生化指标变化剧烈 ,内脏继发损伤更严重。④血浆晶体渗透压升高明显。⑤血管通透性增加持续时间延长 1倍 (达到 16h)。Objective To study the early injury mechanism and characteristic of burn and seawater immersion with rat model.Methods Wistar rat 175, male, weight from 206~290 g. Rat model: rat were made 30% deep Ⅱ° burn on back, immobilized it on plate and immersion rat to seawater except head and arms. Seawater temperature was 20~22℃. Groups: normal contrast n=15, single burn n=50, burn+immersion seawater 2 h n=50, burn+immersion seawater 2 h n=40, burn+immersion seawater 4 h n=20. Observation: death rate, serum osmotic press, blood routine, blood biochemistry. Results Death rate: single burn was 6%, burn plus immersion seawater 2 h 10%, 3 h 25%, 4 h 40%. Serum osmotic press was raised obviously, the longer immersion time the more marked. The changes of the blood biochemistry of the burn with seawater immersion were marked more than general burn, especially Cl -, blood sugar and enzymes. The acid poisoning and the increase of the blood vessel permeability is very serious and persists longer time. Conclusion ① The death rate of the rat with burn and seawater immersion is higher than common burn, the longer immersion time the higher death rate. ② The acid poisoning is very serious and persists longer time. ③ The marker of blood biochemistry have a great change and secondary splanchna damage is move serious. ④ Serum osmotic press raised obviously. ⑤ The increase of consistent time of blood vessel permeability prolongs one times (16 h).
