摘　　要：目的分析我院静脉用药调配中心不合理用药及干预成效，为合理用药提供依据。方法将我院2 0 1 5 年3 月~ 1 2 月P IV A S 审方中记录的不合理用药医嘱进行统计归纳分析及干预成效分析。结果我院主要存在浓度检测、溶媒限制、用法用量、相互作用四类不合理用药情况，但我院干预方式成效明显。结论我院住院患者静脉输液存在部分的不合理用药情况，审方药师可有效地干预不合理用药医嘱，预防及减少不合理用药情况。Objective To analyze irrational prescriptions and intervention in PIVAS providing the basis for the rational use of drugs. Methods From March to December 2015,the irrational prescriptions in PIVAS as recorded in the statistics were conducted with inductive analysis and intervention effectiveness analysis. Results There were four kinds of irrational drug use: concentration detection,solvent limits,usage,dosage and interaction,but intervention was effective in our hospital. Conclusion There are some irrational prescriptions in patients with venous transfusion. The irrational drug prescription can be effectively intervened by prescription- checking pharmacists so as to prevent and reduce adverse events caused by irrational drug use.
