高血压性心脏病与糖尿病性心肌病左心功能差异研究

摘　　要：目的 探讨高血压性心脏病与糖尿病性心肌病患者左心功能差异。方法 选择海口市人民医院2013年8月~2014年12月收治的40例高血压性心脏病患者（高血压性心脏病组）、40例糖尿病性心肌病患者（糖尿病性心肌病组）作为研究对象，并选取同时期本院门诊健康体检者40例作为健康对照组，分别接受常规超声心动图、组织多普勒检查，并进行Tei指数计算，对各组对象测定结果进行统计学分析。结果 高血压性心脏病组、糖尿病性心肌病组室间隔舒张末期厚度、左室后壁舒张末期厚度均高于对照组（P＜0.05），左室射血分数（LVEF）、二尖瓣口血流峰值速度E均低于对照组（P＜0.05），糖尿病性心肌病组二尖瓣口血流峰值速度E低于高血压性心脏病组（P＜0.05）；高血压性心脏病组、糖尿病性心肌病组Tei指数高于对照组（P＜0.05），Ea/Aa低于对照组（P＜0.05）；高血压性心脏病组室间隔Sa、下壁Sa均高于糖尿病性心肌病组和对照组（P＜0.05），室间隔ET、下壁ET均高于糖尿病性心肌病组和对照组（P＜0.05），糖尿病性心肌病组Tei指数高于高血压性心脏病组（P＜0.05）。结论 高血压性心脏病与糖尿病性心肌病均存在左心功能减退，糖尿病性心肌病减退更加显著。Objective To investigate the differences of left ventricular function between diabeticcardiomyopathy and hypertensive cardiopathy. Methods From August 2013 to December 2014, 40 patients withhypertensive heart disease (hypertension heart disease group), and 40 diabetic cardiomyopathy patients (diabeticcardiomyopathy group) in Haikou Municipal People's hospital were included, 40 persons from physical checkupswere also included in healthy control group. Each group received conventional ultrasound Heartbeat figure,tissue Doppler (DTI) examination, and Tei index was calculated. All of the test results were analyzed statistically.Results Interventricular septum thickness (IVST), left ventricular posterior wall thickness (LVPWT) of hypertensiveheart disease group, diabetic cardiomyopathy group were higher than the control group (P<0.05); left ventricularejection fraction (LVEF) and peak velocity of mitral E were lower than the control group (P<0.05); velocity of mitralE of diabetic cardiomyopathy group peak was lower than hypertensive heart disease group (P<0.05); Tei index ofhypertension heart disease group and diabetic cardiomyopathy group were higher than the control group (P<0.05);Ea/Aa of these two groups were lower than the control group (P<0.05); interventricular septum Sa and inferior wallSa of hypertensive cardiomyopathy group were higher than diabetic cardiomyopathy group and the control group(P<0.05); interventricular septum ET and inferior wall ET of hypertensive cardiomyopathy group were higher thandiabetic cardiomyopathy group and the control group (P<0.05); Tei index of diabetic cardiomyopathy group washigher than hypertensive heart disease group (P<0.05). Conclusion Left ventricular dysfunction were observed inhypertensive heart disease and more significant in diabetic cardiomyopathy.
