摘　　要：目的:探讨经乳晕弧形切口治疗男性乳房发育症的优势、适应证和手术方法。方法:选取2005年1月-2015年3月收治的男性乳房发育症患者75例(82)侧为研究对象,将其随机分为2组,其中治疗组38例(43侧),对照组37例(39侧)。治疗组采用经乳晕弧形切口,对照组采用传统开放式切口,即在增生的乳房外上或外下皱襞处作切口。观察比较两组手术时间、术中出血量、切口愈合时间、术后皮下积液、胸壁外形满意度及手术瘢痕满意率等。结果:两组手术顺利,均痊愈出院,无严重并发症。比较两组手术时间、术中出血量、切口愈合时间及术后皮下积液发生率,差异不显著。随访12个月。治疗组胸壁外形满意度及手术瘢痕满意率均高于对照组。结论:经乳晕弧形切口美容效果好,瘢痕不明显,患者满意度高,建议推广。Objective To explore the superiority,indications and surgical technique of periareolar approach in the managementof gynecomastia. Methods 75 cases(82 sides) of gynecomastia in the hospital of mine from January 2015 to March 2015 wereselected as the study objects. The patients were randomly divided into 2 groups. The treatment group had 38 cases (43 sides).The control group had 37 cases(39 sides). The treatment group was given periareolar incision, the control group was giventraditional open surgery. The operation time, amount of bleeding, healing time, subcutaneous hydrops and the satisfaction ofthe chest appearance and surgical scars of two groups were compared. Results The operation was successfully performed inall the 75 patients.No severe operation-related complications were observed. The two groups in the operative time, amount ofbleeding, healing time and subcutaneous hydrops were no significant difference. All patients were followed up for 24 months.The patients of the treatment group were more satisfied with their chest appearance and surgical scars than the patients of thecontrol group. Conclusion Periareolar incision can be better at improving the cosmetic effect and satisfaction of the patients,recommend further promote in clinic.
