额部扩张皮瓣在鼻缺损整复中的应用

摘　　要：目的：探讨应用额部扩张皮瓣修复鼻部分缺损的效果。方法：1985年-2008年，收治15例鼻缺损患者。手术分3期进行：Ⅰ期行额部扩张器置入术皮肤扩张术；Ⅱ期行额部扩张皮瓣转移修复鼻缺损；Ⅲ期行鼻根部皮瓣断蒂修整术。结果：本组15例，3例出现皮瓣远端血运障碍，其中2例转移的额部扩张皮瓣远端坏死。13例患者扩张的额部皮瓣全部成活。随访6个月～23年，再造鼻形态满意，额部供区瘢痕不明显。结论：额部扩张皮瓣修复鼻缺损是简单、可靠的方法。Objective To investigate the effect of repairing nasal defect with the expanded forehead flap. Methods From 1985to 2008,15 patients with nasal defect received treatment.The operation consisted of three stages.The expander was implantedat first stage.The expanded forehead flap were transferred and the nasal defect were repaired at second stage. At the thirdstage,pedicle division was performed. Results 15 cases were performed by this method.In 3 cases, the distal portion of thetransplanted forehead flap occured disorder of blood supplying. Finally,the distal portion of the expanded forehead flap necorsedin 2 cases.In 13 cases,the expanded forehead flap survived completely,post-operative follow-up for 6 months to 23 years showedsatisfactory results with good appearance.The scar of forehead donor site was not obvious. Conclusion Using the expandedforehead flap to repair nasal defect is a simple and reliable method.
