盐酸克林霉素棕榈酸酯分散片治疗急性支气管炎成本-效果分析

摘　　要：目的:对盐酸克林霉素棕榈酸酯分散片和克林霉素片治疗急性支气管炎成本-效果分析,以期为药品定价和促进临床医生合理用药提供参考。方法:使用决策树模型,采用盐酸克林霉素棕榈酸酯分散片与克林霉素片治疗急性支气管炎的临床观察的疗效和成本数据进行成本-效果分析。结果:盐酸克林霉素棕榈酸酯分散片组总成本比克林霉素片组高70.34元。盐酸克林霉素棕榈酸酯分散片组与克林霉素片组相比,以临床痊愈率作为指标,存在成本较好,效果较好的情况,每多获得一个百分点的痊愈率多花费2.54元;以显效率作为指标,也存在成本较好,效果较好的情况,每多获得一个百分点的显效率多花费13.71元;以显效时间作为指标,也存在成本较高,效果较好的情况,每多节省一天显效时间多花费74.04元;以痊愈时间作为指标,也存在成本较好,效果较好的情况,每多节省一天痊愈时间多花费213.15元。结论:比较盐酸克林霉素棕榈酸酯分散片和克林霉素片治疗急性支气管炎多花费的成本,和节省显效和痊愈时间增加工作时间,以2015年全国城镇职工日均工资215.94元作为阈值,盐酸克林霉素棕榈酸酯分散片与克林霉素片相比较,治疗急性支气管炎具有经济性。Objective: Cost-effectiveness analysis was performed thereafter on clindamycin palmitate hydrochloride dispersible tablet and clindamycintabletin treating acute bronchitis so as to provide evidence and suggestions for government drug pricing and offer a reference for the rationaladministration of clinical doctors. Methods: Decision tree model was adopted to analyze cost-effectiveness of clindamycin palmitate hydrochloridedispersible tablet and clindamycin tablet in treating acute bronchitis by using the data of therapeutic effect and treatment cost. Results: Compared withclindamycin tablet group, the cost of clindamycin palmitate hydrochloride dispersible tablet group is lower-cost and more efficacy no matter taking theindex of clinical recovery rate, significant efficiency, initiating time or recovery time into account. As a percent of clinical recovery rate increased thecost will increase 2.54yuan,and as a percent of significant efficiency increased the cost will increase 13.71yuan,the initiating timereduce one day thecost will increase74.04 yuan and the recovery timereduce one day the cost will increase 213.15 yuan. Conclusion: Considering the urban employees'average daily salary cap was 215.94 yuan, clindamycin palmitate hydrochloride dispersible tablet has better cost-effectiveness profile compared toclindamycin tablet in the treatment of acute bronchitis after comparing the additional cost in treating acute bronchitis and additional working time forreducing initiating time and recovery time.
