血液透析联合血液灌流治疗尿毒症顽固性皮肤瘙痒的临床疗效

摘　　要：目的:探讨血液透析联合血液灌流对尿毒症顽固性皮肤瘙痒的治疗效果。方法:收治维持性血液透析患者32例,随机分为两组:血液灌流+血液透析(HP+HD)组16例,每周常规HD治疗2次,HP+HD治疗1次;另一组为单纯的HD,每周常规HD治疗3次,共5周。比较两组患者治疗前后血清甲状腺激素(PTH)、β_2-MG浓度、血Pi浓度及皮肤瘙痒缓解程度。结果:HP+HD组患者皮肤瘙痒的缓解率87.5%,高于HD组的18.75%(P<0.05),HP+HD组血清PTH、β_2-MG浓度、血Pi浓度较治疗前有明显下降,皮肤瘙痒明显缓解(P<0.05),而HD组差异无统计学意义(P>0.05)。结论:血液透析联合血液灌流可以明显缓解尿毒症顽固性皮肤瘙痒。Objective:To explore the treatment effect of hemodialysis combined with hemoperfusion in uremia refractory pruritus.Methods:32 patients with maintenance hemodialysis were selected.They were randomly divided into two groups.16 cases in the hemoperfusion and hemodialysis(HP+HD) group were given regular HD treatment 2 times every week,HP+HD treatment 1 time.The another group was given pure hemodialysis(HD),regular HD treatment 3 times every week,total 5 weeks.The serum thyroid hormone(PTH), β 2-MG concentration,serum Pi concentration and remission degree of pruritus were compared between groups before and after treatment.Results:The remission rate of pruritus in HP+HD group was 87.5%,which was higher than 18.75% of HD group(P<0.05).The serum PTH,β2-MGconcentration,serum Pi concentration was significantly decreased than those before treatment,and the pruritus had significant remission(P<0.05),but HD group had no significant difference(P>0.05).Conclusion:Hemodialysis combined with hemoperfusion can significantly relieve uremia refractory pruritus.
