杂合式血液净化治疗肾功能不全合并晚期血吸虫性肝硬化46例临床分析

摘　　要：目的:讨论肾功能不全合并晚期血吸虫性肝硬化患者用杂合式血液净化的治疗效果。方法:收治肾功能不全合并晚期血吸虫性肝硬化患者46例,将患者随机分为3个组,对照组两组(单超疗法和单纯血液透析疗法)和治疗组(杂合式血液净化疗法),测定3组治疗1个月前、后血ALT、AST、ALB、TBIL、DBIL、BUN、Cr及血Na^+的含量,比较其变化。并观察治疗后半年内3组患者的好转、未愈、复发及死亡。结果:对照组治疗前后各项指标比较,差异无统计学意义(P>0.05);3组治疗后比较,差异有统计学意义(P<0.05)。结论:杂合式血液净化疗法能显著改善临床症状,控制病情发展,是治疗肾功能不全合并血吸虫性肝硬化的一种有效、相对安全的手段。Objective:To discuss the treatment effect of hybrid type blood purification in the treatment of renal insufficiency combined with advanced schistosomiasis cirrhosis.Methods:46 patients with renal insufficiency combined with advanced schistosomiasis cirrhosis were selected.The patients were randomly divided into three groups,the control group(simple blood ultrafiltration therapy and simple hemodialysis therapy),and the treatment group(hybrid type blood purification therapy).The serum ALT,AST,ALB,TBIL,DBIL,BUN,Cr and serum Na+ contents of three groups were measured in 1 month before and after treatment,and the changes were compared.Within the six months after treatment,the improved,not healed,recurrence and death patients of three groups were observed.Results:The indicators of the control group were compared before and after treatment,and the differences were not statistically significant(P>0.05).There were significant differences between three groups(P<0.05).Conclusion:The hybrid type blood purification therapy can significantly improve the clinical symptoms,and control the development of the disease.It is an effective and relatively safe mean in the treatment of renal insufficiency combined with schistosomiasis cirrhosis.
