综合性方法治疗轻度先天性上睑下垂29例临床观察

摘　　要：目的：探讨治疗轻度先天性上睑下垂的综合方法。方法：轻度先天性上睑下垂患者采用离断眶隔脂肪与睑板上缘附近的上睑提肌腱膜存在的增厚的纤维条索，离断纤维条索后可矫正大部分的轻度上睑下垂，如果离断纤维条索后无法完全矫正上睑下垂，再行上睑提肌腱膜前徙缩短术矫正。结果：本组21例行单纯离断增厚的纤维条索术，19例术后上睑下垂症状完全消失，活动自如，外观自然，左右对称；2例术后上睑下垂症状未完全矫正，抬眼得到改善；随访1个月～1年，无复发。8例行离断增厚的纤维条索术+上睑提肌腱膜前徙缩短术，8例术后上睑下垂症状完全消失，活动自如，外观自然，左右对称，随访1个月至1年，无复发。结论：采用离断眶隔脂肪与睑板上缘附近的上睑提肌腱膜存在的增厚的纤维条索和/或上睑提肌腱膜前徙缩短术是可行的方法，并取得满意的效果。Objective To study the comprehensive methods of treatment of mild congenital Ptosis. Methods Most of the mildPtosis patients can be treated by breaking the orbital fat and the thickened fiber cables of levator aponeurosis near the upper edgeof the tarsal plate. If the broken of cables cannot treat Ptosis completely, further shortening of levator aponeurosis former acts isnecessary. Results There were 21 cases operated simply by breaking the thickened fiber cables. Ptosis symptoms completelydisappeared in 19 cases, they had free movements, natural and symmetrical appearances after the surgeries. Although 2 caseswere not treated completely, their eyelids lifting improved. There was no recurrence in a follow-up of 1 month to 1 year afterthe surgery. There were 8 cases operated by breaking the fiber cables and shortening of levator aponeurosis former acts. Ptosissymptoms completely disappeared in all the cases with free movements, natural and symmetrical appearances. There was norecurrence in a follow-up of 1 month to 1 year after the surgery. Conclusions Breaking the orbital fat and the thickened fibercables of levator aponeurosis near the upper edge of the tarsal plate and/or shortening of levator aponeurosis former acts is afeasible method to treat mild congenital Ptosis, and also have achieved satisfactory results.
