腹腔镜下胃癌根治术与开腹胃癌根治术的临床观察

摘　　要：目的通过比较腹腔镜下胃癌根治术与开腹胃癌根治术的临床疗效,明确腹腔镜运用于胃癌根治术的优势。方法 40例行胃癌根治术的患者,根据术式不同分为试验组与对照组,各20例。试验组实施腹腔镜胃癌根治术,对照组实施开腹胃癌根治术。比较两组患者手术时间、术中出血量、术后排气时间及远期生存率情况。结果试验组术中出血量(157.9±13.9)ml、术后排气时间(4.2±1.1)d优于对照组(178.2±15.8)ml、(6.3±1.4)d(P<0.01),两组手术时间及1、3年生存率比较差异无统计学意义(P>0.05)。结论腹腔镜与开腹胃癌根治术后远期疗效不存在明显差异,但腹腔镜相比于开腹具有创伤小、安全性高的优势。Objective To clarify advantages in laparotomy radical operation for stomach cancer, throughcomparing clinical effects between laparoscopic and laparotomy radical operation for stomach cancer. Methods Atotal of 40 patients receiving radical operation for stomach cancer were divided by different operation measures intoexperimental group and control group, with 20 cases in each group. The experimental group received laparoscopicradical operation for stomach cancer, and the control group received laparotomy radical operation for stomachcancer. Comparison was made on operation time, intraoperative bleeding volume, postoperative exhaust time,and long-term survival rate between the two groups. Results The experimental group had better intraoperativebleeding volume as (157.9±13.9) ml and postoperative exhaust time (4.2±1.1) d than (178.2±15.8) mland (6.3±1.4) d in the control group (P<0.01). There was no statistically significant difference of operation timeand 1, 3-year survival rates between the two groups (P>0.05). Conclusion Although there is no remarkabledifference of long-term curative effect between laparoscopic and laparotomy radical operation for stomach cancer,laparoscope still contains advantages of small trauma and high safety.
