双球囊导管联合缩宫素对足月妊娠促宫颈成熟及引产的影响

摘　　要：目的:观察双球囊导管联合缩宫素用于足月妊娠促宫颈成熟及引产的临床效果及安全性。方法:选择住院分娩的初产妇584例,分为观察组(球囊联合缩宫素)、对照1组(单用缩宫素)及对照2组(自然临产并分娩)。比较促宫颈成熟和引产情况、阴道分娩结局,以及产程和母婴并发症等。结果:观察组取出导管时的宫颈Bishop评分,较放置前有非常显著改善(P<0.01),且与对照1组比较差异有统计学意义(P<0.05);观察组阴道分娩率显著高于对照1组(P<0.05),剖宫产率显著低于对照1组(P<0.05);潜伏期和总产程均显著短于对照1组和对照2组(P<0.05);观察组与对照1组产后出血比较差异有统计学意义(P<0.05)。结论:双球囊导管联合缩宫素用于足月妊娠促宫颈成熟及引产,可显著改善宫颈条件,提高阴道分娩率,是一种有效、安全的方法。Objective: To observe the clinical effect and safety of double balloon catheter combined oxytocin on cervical ripening and induction of labor in term pregnancy. Method: 584 cases of childbirth primipara in our hospital were selected and divided into the observation group (balloon combined with oxytocin) and the control group 1 (oxytocin alone) and control group 2 (natural labor and childbirth). The cervical ripening, induced labor and vaginal delivery, as well as labor and maternal and infant complications, etc.of three groups were compared. Result: The Bishop score of the cervix of the observation group upon taking out the catheter was significantly improved (P<0.01), and the difference was statistically significant compared with that of control group 1 (P<0.05); Vaginal births rate of the observation group was significantly higher than that of control group 1 (P<0.05), cesarean section rate was significantly lower than that of control group 1 (P<0.05); The incubation period and the total labor of the observation group were significantly shorter than that of control 1 group and control group 2 (P<0.05); The difference of postpartum hemorrhage of the observation group and control group 1 was statistically significant (P<0.05). Conclusion: Double balloon catheter combined oxytocin, which were used to promote cervical mature and full-term pregnancy induced labor, could significantly improve the conditions of the cervix, enhance childbirth vaginal rate and it was an effective and safe method.
