自拟扶正祛邪汤配合载药自固化磷酸钙人工骨治疗慢性骨髓炎24例

摘　　要：目的：探讨自拟扶正祛邪汤中药内服配合载药万古霉素自固化磷酸钙人工骨（CPC）治疗慢性骨髓炎的临床疗效。方法：2011年3月-2014年4月，对我院骨科24例慢性骨髓炎患者术前采用自拟扶正祛邪汤（根据院内治疗骨髓炎经验方自拟扶正祛邪汤）中药内服，术中行彻底病灶清除冲洗，一期植入自固化磷酸钙人工骨（上海瑞邦生物材料有限公司生产，瑞邦骨泰）载药万古霉素颗粒，一期闭合伤口，均不放置引流。术后继续中药内服治疗。结果：参照1994年国家中医药管理局颁布的《中医病证诊断疗效标准》，本组24例患者术后均获得随访，随访时间12～37个月，平均27.8月。治愈21例，好转2例（经换药后治愈），1例未愈，后行截肢手术，治愈率达87.5%，总体疗效满意。治愈患者无全身反应，未发生再骨折，X线显示随访者CPC大部分降解吸收成骨。结论：中药内服配合载药自固化型磷酸钙人工骨治疗慢性骨髓炎临床疗效确切，容易被患者接受，值得临床推广应用。Objective: To explore clinical efficacy of the Fuzheng Quxie decoction plus drug-impregnated calcium phosphate cementon chronic osteomyelitis. Methods: 24 cases were treated with the Fuzheng Quxie decoction before the operation. In operation the lesionwas completely cleared; drug-impregnated calcium phosphate cement (containing vancomycin was implanted); then the wound was closed,with no placed drainage. After the operation, patients were given TCM medicine. Results: 24 cases were followed up for 12~37 months,average 27.8 months. 21 cases were cured; 2 cases were improved and cured after dressing change; 1 case was not healed and underwentamputation; the cure rate was 87.5%, with satisfied efficacy. Patients were no systemic reaction, no fracture; CPC of follow-up patients wasdegraded and absorbed into bone by X-ray. Conclusion: The Fuzheng Quxie decoction plus drug-impregnated calcium phosphate cement onchronic osteomyelitis shows a curative effect, is easy to be accepted, and worthy of application.
