复合组织瓣移植修复四肢严重创伤后皮肤缺损的临床研究

摘　　要：目的:探讨复合组织瓣移植修复四肢严重创伤后皮肤缺损的临床应用价值。方法:选择我院2012年4月至15年1月收治的92例严重创伤后皮肤缺损患者为研究对象,以随机数字表法分为对照组46例,观察组46例,对照组患者为创面坏死组织溶解、肉芽组织生长后再实施皮瓣修复术;观察组患者采取复合组织瓣移植修复,对两组患者修复效果进行观察。结果:观察组术后肢体恢复优良率为91.30%,明显较对照组71.74%高,两组差异存在统计学意义(P<0.05);观察组手术前后住院时间明显较对照组长,住院费用较低,差异有统计学意义(P<0.05);对照组术后并发症发生率30.43%,观察组10.87%,两组差异显著(P<0.05)。结论:四肢严重创伤后皮肤缺损患者实施复合组织瓣移植修复,可利于术后肢体功能恢复,缩短住院时间及住院费用,且术后并发症较少,能减轻患者痛苦,提高满意率,值得推广。Objective To investigate the clinical value of composite tissue flap in the repair of skin defect after severe trauma.Methods 92 patients of skin defect caused by severe trauma in our hospital from April 2012 to January 2015 were selectedas the research object and were randomly divided into control group(46 cases) and observation group(46 cases),control groupimplemented flap repair surgery after necrotic tissue dissolution and growth of granulation tissue, patients in the observationgroup were treated with composite tissue flap transplantation,the repair effect of the two groups were observed. Results Theexcellent and good rate of limb recovery after operation in the observation group was 91.30%,significantly higher than thatin the control group,the difference was statistically significant between the two groups(P <0.05).Compared with the controlgroup,the hospitalization time of the observation group was significantly lower than that of the control group,and the differencewas statistically significant(P <0.05).The incidence rate of postoperative complications was 30.43% in the control group,10.87%in the observation group,the difference between the two groups was significant(P <0.05). Conclusion Patients with skin defect repair flap after severe trauma of limbs,can facilitate postoperative recovery of limb function,shorten the hospitalization cost andhospitalization time,and less complication,can relieve the pain of patients,improve the satisfaction rate,worthy of promotion.
