鼻中隔软骨及耳廓软骨在鼻延长中的灵活应用

摘　　要：目的：探讨在不同鼻中隔先天条件和鼻中隔软骨片切取大小情况下，灵活选用不同的鼻中隔软骨与耳软骨处理技术组合完成鼻延长的方法。方法：通过开放入路，松解下外侧软骨，常规方法显露鼻中隔软骨，根据不同情况选用保留L支架的鼻中隔软骨延伸移植技术、不保留L支架的鼻中隔翻转成形技术、耳软骨鼻尖移植物、鼻中隔软骨鼻小柱支撑移植物、耳软骨复合鼻中隔软骨鼻小柱支撑移植物、耳软骨穹窿重建移植物等技术相组合，并结合下外侧软骨缝合技术完成短鼻延长。结果：本组51例短鼻患者，随访其术后3～15个月，7例患者鼻延长效果欠佳，余44例患者短鼻改善明显。结论：在开放式鼻整形中根据患者鼻中隔先天条件及获取鼻中隔软骨移植片大小采取不同的鼻中隔软骨与耳软骨处理技术组合。Objective To explore the flexible strategy for lengthening short nose with septal cartilage and auricular cartilage,in the case of different congenital characteristics of septum and the sizes of the harvested septal cartilage. Methods Withthe open approach,released lower lateral cartilage,exposed the septum cartilage by routine method.According to the variouscongdition,septum extension graft with the reserved L-shaped septum cartilage structure , somersault septoplasty without thereserved L-shaped septum cartilage structure, auricular cartilage tip graft,septal cartilage collumella strut, collumella strutcombined with septal cartilage and auricular cartilage ,and domal reconstruction graft were chosed,then lateral crus sutureplaced. Results 51 cases were followed up for 3 to 15months.7 cases presented deficient effect ,other 44 cases presentedfavorable effct. Conclusions In the open rhinoplasty,according to different congenital characteristics of septum and the sizesof the harvested septal cartilage,choosing different techniques involving with septal cartilage and auricular cartilage,whitplacing the lateral crus suture.effective nose elongation may achieve.
