摘 要:目的:研究中心静脉导管(central venous catheter,CVC)胸腔闭式引流(thoracic close drainage,TCD)(CVCTCD)与改良胸腔闭式引流(Modified thoracic closed drainage,M-TCD)治疗闭合性气胸的临床疗效。方法:选取112例闭合性气胸住院患者,根据治疗方式的不同分为CVC-TCD 61例(中心组)和M-TCD 51例(改良组),并进行观察比较。结果:两组患者肺复张时间差异均无统计学意义(P>0.05);中心组患者术后疼通指数评分较改良组低,差异有统计学(P<0.05);中心组患者拔管时间、住院时间、住院费用均较改良组短,有统计学差异(P<0.05);中心组患者并发症发生较改良组少(P<0.05)。结论:CVC-TCD与M-TCD在治疗自发性及外伤导致的闭合性气胸上具有同样的疗效,且CVC-TCD相对于M-TCD具有并发症少、带管和住院时间短、及花费少的优势。Objective: To study the effect of center venous catheter (central venous catheter CVC) closed thoracic drainagepleural (thoracic close drainage (TCD) CVC-TCD and meliorate closed drainage modified thoracic closed drainage M-TCDin treatment of closed pneumothorax.Methods:112 cases of hospitalized patients with closed pneumothorax,according to thedifferent treatment methods were divided into CVC-TCD 61 cases (central group) and M-TCD 51 cases (improved group),andobserved and compared.Results:Two groups of patients with pulmonary reexpansion time differences had no statisticalsignificance (P>0.05); central group of patients after pain score index than in the modified group,difference was statistically(P<0.05); group extubation time,hospitalization time,cost of hospitalization were compared with modified group,there wasstatistically significant difference (P<0.05); center groups of patients with complications occurred than in the modified group(P<0.05).Conclusion:CVC-TCD and M-TCD in the treatment of spontaneous and trauma caused by closed pneumothorax hasthe same efficacy,and CVC-TCD relative to M-TCD with fewer complications,with tube nd shorter hospital stay,and less costadvantage.
