摘　　要：目的:针对我国的老龄化现状,提出了面向老年健康管理的临床护理路径的必要性,探索实现的模式与方法。方法:围绕老年健康管理的基本原则,阐述了老年临床护理路径建设的必要性,以及从路径的创建、跟踪与优化3个方面开展老年临床护理路径的方法。结果:构建科学合理而富有中国特色的老年健康临床护理路径管理系统。结论:通过临床路径的推广与应用必将实现健康资源的合理分配,提高基层医护人员的诊疗水平,保障老年人享有公平的医疗资源。Objective: In view of the present situation of aging in our country, it puts forward the necessity of the clinical nursing pathfor the elderly health management, and explores the mode and method of the realization. Method: According to the basic principlesof the health management of the elderly, the necessity of the construction of the clinical nursing path is expounded, and the ways tocarry out the clinical nursing path from 3 aspects of the path creation, tracking and optimization are discussed. Result: Constructionof scientific and rational and full of Chinese characteristics of the elderly health clinical nursing path management system. Conclusion:Through the promotion and application of clinical pathway, it is sure to realize the rational distribution of health resources, improvethe level of diagnosis and treatment of primary health care workers, and protect the elderly to enjoy the fair medical resources.
