重型颅脑损伤患者早期气管切开对肺部感染与预后的影响分析

摘　　要：目的:研究探讨重型颅脑损伤患者早期气管切开对肺部感染与预后的影响。方法:选取我院2014年3月—2015年12月收治的重型颅脑损伤患者80例作为研究对象,并按照患者伤后气管切开的时间将其分为两组,伤后24 h内、24 h后行气管切开处理的患者各有40例,分别设为观察组和对照组,对两组患者治疗后的肺部感染发生情况、感染的控制效果及患者最终的预后情况等进行统计和比较。结果:观察组和对照组患者的肺部感染发生率分别为30.0%和87.5%,感染控制率分别为91.7%和51.4%,感染控制时间分别为(7.1±1.1)d和(13.9±2.0)d,病死率分别为10.0%和47.5%,均有观察组患者显著优于对照组患者的情况,差异有统计学意义(P<0.05)。结论:重型颅脑损伤患者行早期气管切开有利于肺部感染发生率的降低和预后改善,值得临床推广应用。Objective: To study the effect of early tracheotomy on pulmonary infection and prognosis in patients with severe traumatic brain injury. Methods: 80 patients with severe traumatic brain injury, treated in our hospital from March 2014 to December 2015,were selected as research subjects and randomly divided into two groups according to the time of incision in patients after injury, 40 patients received incision 24 hours within injury were in the observation group and 40 patients received incision 24 hours after injury were in the control group, the pulmonary infection, the control effect of infection and the prognosis of patients of two groups after treatment were statistically collected and compared. Results: The pulmonary infection of the observation group and the control group was 30.0% and 87.5% respectively; the control rate of infection of the observation group and the control group was 91.7% and 51.4% respectively; the infection control time of the observation group and the control group was (7.1±1.1) d and (13.9±2.0) d respectively; the fatality rate was 10.0% and 47.5% respectively; the situation of the observation group was significantly better than that of the control group, and the difference was of statistical significance (P<0.05). Conclusion: Early tracheotomy in patients with severe traumatic brain injury helped to decrease the pulmonary infection rate and improve the prognosis and it was worth clinical promoting.
