疼痛护理在小儿先天性心脏病手术后的应用

摘　　要：目的:探讨疼痛护理在小儿先天性心脏病手术后的应用效果。方法:收治小儿先天性心脏病手术后患儿60例,随机平分两组,对照组在躁动期给予安定、吗啡、维库溴铵注射液进行镇静。观察组给予咪唑安定,吗啡静脉持续泵入,用药过程床位护士及时进行FLACC疼痛评分和Ramsay镇静评分,严密监测生命体征、中心静脉压等,根据情况适当调整持续泵入药物的剂量。加强呼吸系统的监护、观察药物不良反应。结果:观察组患儿术后6 h、12 h、24 h心率、平均动脉压、呼吸频率明显优于对照组;观察组发生术后肺不张1例,对照组发生4例,两组比较,差异有统计学意义(P<0.05)。结论:加强呼吸系统的监测、药物不良反应的观察,患儿术后生命体征比较稳定,减少了患者并发症的发生。Objective:To explore the effect of pain nursing in children with congenital heart disease after operation.Methods:60children with congenital heart disease after operation were selected,they were randomly divided into the two groups on average,thecontrol group was sedated with sedative,morphine and vecuronium injection during restless period,while the observation groupwas given midazolam, morphine vein continuous pumping,in the course of medication,the bed nurses mannered FLACC pain scoreand Ramsay sedation score timely,close monitoring of vital signs,central venous pressure,and so on,adjust the dose of continuouspumping drugs according to the situation.Strengthen the monitoring of respiratory system and observe the adverse drug reaction.Results:The heart rate,mean arterial pressure,respiratory rate in the observation group were significantly better than those in thecontrol group at 6 h,12 h,24 h after operation.In the observation group,there was 1 case of postoperative lung injury and 4 cases inthe control group,the differences between the two groups were statistically significant(P<0.05).Conclusion:Strengthen themonitoring of respiratory system,drug adverse reaction observation,the postoperative vital signs are relatively stable,which canreduce the incidence of complications.
