自我管理对晚期鞘内镇痛植入癌痛患者生活质量的影响

摘　　要：目的:探讨自我管理教育对晚期癌痛患者鞘内镇痛输注系统植入术后生活质量的影响。方法:选择60例晚期癌痛行鞘内镇痛输注系统植入术患者,随机分为试验组和对照组,每组30例,对照组按常规护理教育,试验组进行常规护理教育及自我管理教育。采用疾病知识调查表、自我管理能力评分表和生活质量评价表测评两组患者出院时、出院后1月和3月进行测评的相关状态。结果:试验组患者对疾病知识了解程度、自我管理能力及生活质量均优于对照组,差异有统计学意义(P<0.05)。结论:自我管理教育有助于晚期癌痛患者行鞘内镇痛输注系统植入术后建立全面的院外自我管理模式,提高其自我管理能力及生活质量。Objective: To investigate the effects of self-management education on the quality of life in patients with implanted programmable continuous intrathecal morphine infusion pump to relieve advanced cancer pain. Methods: A total of 60 patients with terminal cancer pain were implanted a programmable device for continuous morphine infusion and were randomized into control and test group (30 cases for each group).Patients in control group received usual nursing care, while patients in test group received usual nursing care plus self-management training simultaneously. The questionaire of disease knowledge, self-management ability score table, together with the evaluation forms of life quality were selected to judge the status when patients were discharged from hospital, as well as at 1 and 3 months post-discharge. Results: Compared to control group, there were significant improvements in test group in disease knowledge, self-management ability and quality of life (P < 0.05). Conclusion: For patients with implanted programmable continuous intrathecal pump against advanced cancer pain, self-management education could contribute to set up a healthy hospital discharge self-management pattern to enhance the ability of self-management and improve the quality of life.
