经闭孔尿道中段悬吊术治疗压力性尿失禁及并发症防治探讨

摘　　要：目的：探讨经闭孔无张力尿道中段悬吊术(TVT-O)治疗女性压力性尿失禁的临床价值。方法：采用法国通用公司生产的网片，自制成TVT-O蝶形吊带，对压力性尿失禁患者30例施行TVT-O手术。结果：手术时间35～45min，术中出血100～150mL。拔管后尿失禁消失27例，咳嗽增加腹压后偶有尿液溢出3例，阴道切口网片外露2例，患者发生尿潴留2例。随访6个月，无一例复发。结论：自制吊带系统TVT-O手术操作简捷，手术疗效确切，创伤小，患者恢复快，价格便宜。Objective:To investigate the clinical value of tension free vaginal tape-obturator system(TVT-O)in the treatment of female stress urinary incontinence.Methods:Using the French General company production net piece,made into TVT-O butterfly sling,TVT-O operation was performed in 30 patients with stress urinary incontinence.Results:The operation time was 35～45 min,and the intraoperative bleeding was 100～150 mL.Urinary incontinence disappeared after extubation in 27 cases,3 cases with occasional urine overflow caused by cough increase abdominal pressure,vaginal incision mesh exposed in 2 cases,urinary retention occurred in 2 patients.All cases were followed up for 6 months,no recurrence.Conclusion:Self made sling system TVT-O operation is simple;the operation effect is exact;the wound is small;the patient restores quickly;the price is cheap.
