长脉宽Nd:YAG激光和口服特比奈芬联合疗法与单一治疗甲真菌病临床疗效比较研究

摘　　要：目的:比较长脉宽Nd:YAG 1 064nm激光及口服特比萘芬单独疗法和联合治疗甲真菌病的临床疗效和安全性。方法:临床入组53例甲真菌病患者(共90个病甲),将之随机分为三个治疗组,包括:T组:单独予以每日口服250mg特比萘芬;L组:每周接受1次长脉宽Nd:YAG 1 064nm激光治疗;T+L组:每日口服250mg特比萘芬及每周接受1次长脉宽Nd:YAG1 064nm激光治疗。在治疗后4、8、12、16及24周评估真菌学治愈率及临床治愈率。结果:随着时间推移,真菌学治愈率及临床治愈率均随之提高。治疗8周、12周、16周及24周后T+L组的MCR和CCR均明显高于T组和L组。结论:长脉宽Nd:YAG1 064nm激光及口服特比萘芬联合治疗可迅速有效地治疗甲真菌病。Objective To compare the clinical effect of combined therapy or monotherapy of long-pulsed 1 064nm Nd:YAGlaser and oral terbinafine for onychomycosis. Methods 53 patients with 90 infected nails were randomly divided into threegroups. T group and L group received oral terbinafine and long-pulsed Nd:YAG laser treatment respectively, and T+L groupreceived combined therapy. The mycological clearance rate (MCR) and the clinical clearance rate (CCR) of the three groupswere evaluate at different time-points. Results With the increased duration of treatment, the MCR and CCR increasedin three groups. The MCR and CCR of the T+L group were significantly higher than those of T group and L group after 8weeks. Conclusion Combined therapy of long-pulsed 1 064nm Nd:YAG laser and oral terbinafine are more effective foronychomycosis.
