恩替卡韦对慢性乙型肝炎患者外周血T细胞亚群和肝纤维化指标的影响

摘　　要：目的探讨慢性乙型肝炎患者采取恩替卡韦治疗对肝纤维化指标及外周血T细胞亚群的影响。方法选取2014年3月~2015年3月期间接收的52例慢性乙肝患者作为试验组,另选择42例正常健康体检者作为对照组,试验组采用恩替卡韦治疗,观察疗效。结果与对照组比较,试验组外周血CD8+水平增加,CD4+水平减少,CD4+/CD8+水平下降,差异均有统计学意义(P<0.05);治疗6个月后,患者上述指标均显著改善(P<0.05),HBV-DNA含量较疗前明显下降(P<0.05);肝纤维化指标PCⅢ、HA、LN、Ⅳ-C在治疗6个月后均较疗前显著下降(P<0.05)。结论外周血T细胞亚群紊乱是慢性乙肝患者的病理表现之一,临床治疗采用恩替卡韦药物疗法可调节外周血T细胞亚群紊乱,改善患者肝纤维指化指标,抑制HBV-DNA病毒复制,疗效显著。Objective To investigate the chronic hepatitis b patients taking entecavir therapy for liver fibrosis indexes and the influence of peripheralblood T cell subsets.Methods Selecting52cases from March2014to March2015men of our hospital during the period of receiving as experimental group,patients with chronic hepatitis b(CHB)another choose42cases of normal healthy physical examination as control group,experimental group use entecavirtreatment,observation curative effect.Results compared with control group,experimental group and an increased level of peripheral blood CD8+,CD4+levels decrease,CD4+/CD8+levels drop.Six months after treatment,patients with the above indicators were significantly improved(P<0.05),HBV-DNA content than before treatment significantly decreased(P<0.05);Liver fibrosis index PCⅢ,HA,LN,Ⅳ-C in the treatment of6months after thanbefore treatment significantly decreased(P<0.05).Conclusion Peripheral blood T cell subsets disorder is one of the pathological manifestation of patientswith chronic hepatitis b(CHB),clinical treatment with entecavir drug therapy adjustable peripheral blood T cell subsets disorder,improve patient liver fiber tochange index,inhibit HBV DNA replication,curative effect is distinct,the value for popularization.
