硬膜外超前镇痛应用于胃癌患者的围术期应激反应研究

摘　　要：目的：探究硬膜外超前镇痛应用于胃癌患者的围术期应激反应的影响。方法：选取2011年3月～2014年3月我院收治的82例行胃癌根治术患者，随机分为观察组和对照组两组，每组各41例，对照组患者给予术后硬膜外镇痛，观察组患者给予硬膜外超前镇痛，观察比较两组患者术后的皮质醇、促肾上腺皮质激素、C反应蛋白及疼痛视觉VAS评分。结果：治疗后，两组患者的血清中的皮质醇、促肾上腺皮质激素、C反应蛋白均较治疗前有明显升高，对照组血清中的皮质醇、促肾上腺皮质激素、C反应蛋白较观察组升高明显，组间比较差异明显，具有统计学意义（P＜0.05）。结论：对胃癌围手术期患者给予硬膜外超前镇痛，疼痛缓解效果明显，对围手术期间的应激反应具有较强抑制作用，安全性髙。Objective: To explore the effectof preemptive epidural analgesia in perioperative stress response in patients with gastric cancer.Methods:82 gastric cancer patients from March 2011 to March 2014 in our hospital from underwent radical gastrectomy were sellected and randomly divided into observation group and control group,with 41 cases in each group,the control group were treated with postoperative epidural analgesia,patients in the observation group were given epidural and preemptive observe the pain were compared between the two groups of patients after cortisol,adrenocorticotrophic hormone,Creactiveprotein and pain visual VAS score were compared.Results:The results after the treatment,cortisol levels in the serum oftwo groups of patients,promote adrenal cortical hormone,C-reactive protein (CRP) treatment and there was a Increased,controlgroup,the serum levels of cortisol,adrenocorticotrophic hormone,C reactive protein than in the observation group were significantly increased,groups significant differences,with were statistically significant(P<0.05).Conclusion:For gastric cancer during perioperative patients given epidural and preemptive pain and pain relief effect is obvious,has a strong inhibitory effect on perioperative stress response,ANN Full of high,it is worth popularizing in clinical use.
