摘　　要：目的系统评价不同卧位对患者中心静脉压测量的影响。方法计算机检索Cochrane Library、MEDLINE、EMbase、中国生物医学文献服务系统、中国学术期刊全文数据库以及万方数据库中公开发表的相关文献,由3名研究者对文献进行评鉴,对纳入研究使用Rev Man 5.3软件进行Meta分析。结果共纳入7篇文献,涉及406例患者。Meta分析显示:30°半卧位时测量中心静脉压值与平卧位时测量中心静脉压值间具有差异(WMD=-0.79,95%CI:-1.39^-0.18,P<0.01);45°半卧位时测量中心静脉压值与平卧位时测量中心静脉压值间具有差异(WMD=-0.96,95%CI:-1.30^-0.61,P<0.001)。结论患者可维持原30°半卧位进行中心静脉压值监测;在维持45°半卧位情况下可结合病情及中心静脉压值,指导下一步治疗及护理策略。Objective To review systematically the influence of different lying positions on monitoring ofcentral venous pressure(CVP).Methods The databases of Cochrane Library,MedLine,EM base,CBM,CNKIand W anFang Database were retrieved with computer for collecting relevant published literature,and the literaturewas reviewed by3researchers.The concluded studies were given a M eta-analysis by using RevMan5.3software.Results There were totally7studies included involved406cases.The results of M eta-analysis showed thatCVP value detected when patients were in30.hem i-recum bent position was different to that detected whenpatients were in supine position(W M D=-0.79,95%C I:-1.39^-0.18,P<0.01),and CVP value detected whenpatients were in45.hem i-recum bent position was different to that detected when patients were in supine position(W M D=-0.96,95%C I:-1.30^-0.61,P<0.001,I2=45%).Conclusion W hen CVP is monitored,patients cankeep30.hem i-recum bent position.W hen patients are in45.hem i-recum bent position,they can be givenfurther therapeutic and nursing suggestions based on disease condition and CVP value.
