脉管复康片治疗下肢动脉硬化性闭塞症疗效和安全性的系统评价

摘　　要：目的评价脉管复康片治疗下肢动脉硬化性闭塞症(LASO)的疗效和安全性。方法计算机检索Pub Med、EMbase、The Cochrane Library(2016年第2期)、CBM、CNKI、Wan Fang Data和VIP数据库,搜集使用脉管复康片治疗LASO患者的相关临床试验,检索时限均为从建库至2016年3月。由两名评价者独立进行筛选文献、提取资料和风险偏倚评价,然后采用Re Man 5.3软件绘制森林图,并系统评价各研究的结果。结果共纳入4个研究,共317例患者。结果表明,脉管复康片优于复方丹参片(总有效率:RR=1.27,95%CI:1.05~1.54),与通塞脉片(RR=1.25,95%CI:0.99~1.58)和寒痉汤联用微汗法(RR=0.83,95%CI:0.66~1.04)疗效相当,脉管复康片联合前列地尔优于单用前列地尔(RR=1.29,95%CI:1.05~1.60)。与其他治疗方案比较,单用或联用脉管复康片组能显著改善患者的临床症状和体征、血流流变学及踝肱指数等,均无严重不良反应发生。结论当前证据表明,与其他治疗方案相比,单用或联用脉管复康片能显著改善LASO患者的临床症状和体征。由于纳入研究的数量和质量的限制,该结论尚需进一步开展相关研究加以验证。Objective To review the curative effect and safety of Maiguanfukang Pian in treatment of lower limb arteriosclerosis obliterans (LASO). Methods The databases of PubMed, EMbase, Cochrane Library (2016.2),CBM, CNKI, WanFang Data and VIP Database were retrieved for collecting clinical trials related to treatment of LASO with Maiguanfukang Pian up to Mar. 2016. The literature was screened and data was extracted by 2 reviewers independently, and the forest plot was drawn by using ReMan 5.3 software and the results of included trials were reviewed systematically. Results There were totally 4 trials included involving 317 patients. The results showed that curative effect of Maiguanfukang Pian was superior to that of Fufangdanshen Pian (total effective rate: RR=1.27,95%CI: 1.05~1.54), and was the same as that of Tongsaimai Pian (RR=1.25, 95%CI: 0.99~1.58) and Hanjing Tang combining Slight sweating method (RR=0.83, 95%CI: 0.66~1.04). The curative effect of Maiguanfukang Pian combining alprostadil was superior to that of single alprostadil (RR=1.29, 95%CI: 1.05~1.60). Maiguanfukang Pian single or combining with other drugs could significantly relieve clinical symptoms and signs, and improve hemorrheology and ankle-brachial index without severe adverse reactions compared with other therapeutic schedules. Conclusion The current evidence shows that Maiguanfukang Pian single or combining with other drugs can significantly relieve clinical symptoms and signs in with patients LASO. As the limitation of quantity and quality of included trials, further relevant studies are needed to confirm the conclusion.
