老年高血压病患者动态脉压指数与胱抑素C、左室质量指数相关性研究

摘　　要：目的探究老年高血压病患者24 h动态脉压指数与胱抑素C、左室质量指数的相关性。方法选取2015年1月~2016年1月于华北理工大学附属医院老年病科门诊及住院的老年高血压病患者,所入选患者完善24 h动态血压监测,依据24 h动态血压计算动态脉压指数(PPI):PPI=24 h动态脉压差/24 h平均收缩压,取PPI≤0.40为A组,PPI在0.41~0.50为B组,PPI≥0.51为C组。检测入选患者血清胱抑素C(Cys C),完善心脏彩超检查并计算左室质量指数(LVMI)。结果 A组、B组、C组,三组间体质指数(BMI)、总胆固醇(TC)、尿素氮(BUN)有升高的趋势,心率(HR)有下降的趋势,但差异无统计学意义(P>0.05)。A组、B组、C组,三组间Cys C、LVMI呈明显递增趋势,差异有统计学意义(P<0.05)。Cys C、左室质量指数(LVMI)均与PPI呈正相关r分为0.923,0.890(P<0.05),与其它指标无明显相关性(P>0.05)。结论老年高血压病患者24 h动态脉压指数与Cys C、LVMI正相关,其中Cys C较LVMI与24 h动态脉压指数相关性更大。Objective To explore the relationship between the 24-hour ambulatory pulse pressure index with cystatin C (Cys C) and left ventricular mass index (LVMI) in elderly patients with essential hypertension. Methods The elderly hypertensive patients of Geriatrics Department of the Affiliated Hospital of North China University of Science & Technology from January, 2015 to January, 2016 were selected and given 24-hours ambulatory blood pressure monitoring to calculate the dynamic pulse pressure index (PPI) on the basis of 24-hour ambulatory blood pressure: PPI=24-hour dynamic pulse pressure/24-hour mean systolic blood pressure. Patients with PPI≤0.40 were enrolled as group A, patients with PPI 0.41-0.50 were enrolled as group B, patients with PPI≥0.51 were enrolled as group C. Cys C and LVMI were measured and calculated. Results Body Mass Index (BMI),total cholesterol (TC) and blood urea nitrogen (BUN) of the three groups had a tendency to increase, and HR has a downward trend, but the difference was not statistically significant (P>0.05). The Cys C and LVMI of the three groups showed an increasing trend, the difference was statistically significant (P<0.05). Cys C and LVMI were positively correlated with PPI (r=0.923, 0.890, P<0.05), and had no significant correlation with other indexes (P>0.05). Conclusion 24-hour ambulatory PPI was positively correlated with Cys C and LVMI in elderly hypertensive patients, and Cys C was more correlated with LVMI and 24-hour ambulatory PPI.
