摘  要:目的:对比研究高频超声、低频超声与磁共振成像(MRI)诊断新生儿颅脑病变的价值,为新生儿脑白质病变选取方便和准确的诊断技术提供参考。方法:选取1055例新生儿行高频超声、低频超声及MRI检查,以MRI检查结果为金标准,分析超声在新生儿颅脑疾病诊断中的检出率、敏感性及特异度等情况。结果:早产儿与足月儿超声诊断阳性率差异显著,有统计学意义(x^2=307.856,P<0.05),早产儿超声诊断阳性率显著高于足月儿。3种不同方法脑白质病变检出率存在差异(x^2=622.332,P<0.05),低频检出率最低,其次为高频,高低频联合和MRI检出率差异不显著,检出率最高。结论:高低频联合成像与MRI检查对新生儿脑白质病变的检出率差异不显著,但MRI技术成本高,而颅脑超声波检查无辐射、操作简单、价格低廉、可重复性强,适宜在基层医院推广应用。Objective: To assess the value of high-frequency ultrasound, low-frequency ultrasound and magnetic resonance imaging (MRI) in the diagnosis of neonatal cerebral disease,and provide reference for choosing convenient and accurate diagnostic methods of neonatal cerebral white matter lesions. Methods: The study has assessed 1055 cases, and these patients were detected by the high-frequency ultrasound, low-frequency ultrasound and MRI (MRI results asthe gold standard). The detection rate, sensitivity and specificity of ultrasound in the diagnosis of neonatal cerebral disease were analyzed. Results: The ultrasonic diagnosis positive rate in premature infant was significantly higher than that infull term infant(x2=307.856, P<0.05). There were significant differences of cerebral white matter lesion detection rates between these three methods(x2=622.332, P<0.05). And the detection rate of low-frequency ultrasonic was lowest, followedby high frequency ultrasound. There was no statistically significance between high-frequency ultrasound combination oflow-frequency ultrasound and MRI in the detection rate, and it’s detection rates were highest. Conclusion: High-frequency ultrasound combination of low-frequency ultrasound and MRI in the diagnosis of neonatal cerebral white matter lesions had similar detection rates. In view of the low cost, no radiation, repeatability and simple operation of cerebral ultrasound, this method is worth to be recommended.
