探索支气管镜肺泡灌洗术用于呼吸机相关性肺炎治疗的效果

摘　　要：目的探讨支气管镜肺泡灌洗术用于呼吸机相关性肺炎治疗的效果。方法 90例呼吸机相关性肺炎患者,随机分为观察组和对照组,各45例。对照组采用基础性治疗,观察组在此基础上行床旁支气管镜吸痰和肺泡灌洗术治疗。比较两组临床症状消失时间、治愈率、住院时间。结果观察组退热时间为(5.0±1.0)d、咳嗽咳痰消失时间为(20.0±0.8)d、肺部啰音消失时间为(15.0±1.8)d、抗生素使用时间为(16.0±2.9)d、住院时间为(15.0±3.0)d、治愈率为95.56%(43/45),均优于对照组的(25.0±3.5)d、(38.5±6.5)d、(25.0±3.8)d、(28.5±5.0)d、(35.0±5.5)d、55.56%(25/45),差异具有统计学意义(P<0.05)。结论支气管镜肺泡灌洗术用于呼吸机相关性肺炎治疗的效果显著,能够有效地促进临床症状改善,缩短治疗时间,提高治疗效果,值得临床广泛应用以及推广。Objective To investigate effect by bronchoalveolar lavage in the treatment of ventilatorassociatedpneumonia.Methods A total of90patients with ventilator-associated pneumonia were randomlydivided into observation group and control group,with45cases in each group.The control group received basictherapy,and the observation group received additional bedside bronchoscopic sputum suction and alveolar to basictherapy.Comparison was made on clinical symptom disappearance time,cure rate and hospital stay time betweenthe two groups.Results The observation group had fever subsidence time as(5.0±1.0)d,cough and sputumdisappearance time as(20.0±0.8)d,lung rale disappearance time as(15.0±1.8)d,antibiotics administrationtime as(16.0±2.9)d,hospital stay time as(15.0±3.0)d and cure rate as95.56%(43/45),which were all betterthan(25.0±3.5)d,(38.5±6.5)d,(25.0±3.8)d,(28.5±5.0)d,(35.0±5.5)d,and55.56%(25/45)in the controlgroup.Their difference had statistical significance(P<0.05).Conclusion Implement of bronchoalveolar lavageshows excellent effect in the treatment of ventilator-associated pneumonia.This method can effectively accelerateclinical symptom improvement,shorten treatment duration,and improve curative effect.It is worth widely clinicalapplication and promotion.
