摘  要:目的探讨于良性阵发性位置性眩晕(Benign Paroxysmal Positional Vertigo,BPPV)的临床诊断中内耳MR水成像的使用价值。方法选取2014年4月~2015年7月间于我院治疗的BPPV患者共78例,分别对其行颞骨高分辨率CT、及内耳MR水成像技术进行检查,观察对比两种方法的诊断价值。结果 CT表现为耳石脱落、半规管受累及听小骨损坏、消失,检查总阳性率为65.38%,内耳MR水成像表现为耳石脱落、半规管变性、半规管破裂及壶腹嵴变薄等,检查总阳性率为89.75%,两者对比CT检查总阳性率明显较低,差异显著(P<0.01)。结论于BPPV的临床诊断中,内耳MR水成像检出率较高,可做为临床评价患者病情及选择治疗方案的有利依据,值得进一步推广使用。Objective To investigate the use value of inner ear liquid Magnetic Resonance Imaging(MRI) in the clinical diagnosis of Benign Paroxysmal Positional Vertigo (BPPV). Methods Seventyeight patients, who were treated in our hospital from April 2014 to July 2015, were examined by high resolution Computed Tomography (CT) of temporal bone and inner ear liquid MRI respectively, so as to observe and compare the diagnostic value of such two methods. Results High resolution CT showed the otolith detachment, semicircular canal involvement as well as ossicles damage and disappearance in 51 patients with the total positive rate of 65.38%. Meanwhile, inner ear liquid MRI found otolith detachment, semicircular canal degeneration, semicircular canal rupture and epithelium thinning in crista ampulla and so on in 71 patients with the total positive rate of 89.75%. Thus the total positive rate of high resolution CT was significantly lower than that of inner ear liquid MRI, and the difference was significant(P <0.01). Conclusion Compared with high resolution CT, the detection rate of inner ear liquid MRI was higher in the clinical diagnosis of BPPV, which could be used as a beneficial basis for clinical evaluation of the patient’s condition as well as choice of treatment plan, and is also worth further promotion in clinic.
