摘　　要：目的:探讨经皮子宫动脉栓塞术(UAE)预防剖宫产术后子宫疤痕妊娠(CSP)清宫术中大出血的临床价值。方法:选取2013年1月—2015年12月连云港市第二人民医院及连云港市妇幼保健院收治的剖宫产术后子宫瘢痕妊娠患者20例,采用UAE栓塞子宫动脉远端,分析治疗后临床效果改善情况。结果:20例CSP患者全部栓塞成功,栓塞后48~72 h行清宫术,清宫过程顺利,术中出血量约30~150mL,均未出现大出血,术后病理均见绒毛组织。β-人绒毛膜促性腺激素(β-HCG)值降至正常时间为10~29 d,平均(16.7±4.5)d。随访3~6个月无再次大出血。结论 UAE是一种能有效预防CSP清宫术大出血的方法,操作方便,疗效明确,值得临床推广。Objective:To investigate the efficacy of uterine artery embolization(UAE)in preventingacute massive hemorrhage caused by uterine curettage with cesarean scar pregnancies(CSP).Methods:20casesof cesarean scar pregnancy admitted into our hospitals from January2013to December2015were selected in thisstudy.All the20patients were treated by UAE.Then the therapeutic effect of UAE on cesarean scar pregnancy wasobserved.Results:20patients were all successfully treated with UAE,and the curettage procedures were performedin next48~72h.Intraoperative blood loss was about30~150ml.Pathological examinations detected villi tissue aftercurettage.Theβ-HCG value resumed to normal levels in10~29d,with an average of(16.7±4.5)d.There were nohemorrhage observed in the follow-up of3~6months later.Conclusions:UAE is an effective therapeutic methodin preventing acute massive hemorrhage caused by uterine curettage with CSP.It is easy to manipulate with evidentbenefit to patients,thus is worthy of clinical application
