摘  要:目的通过MNA-SF评估新泾社区老年人营养状况,分析该社区老年人营养状况与肌肉减少症相关性,并观察营养干预的效果。方法随机抽取上海新泾社区65岁以上老年人300例,通过MNA-SF将新泾社区老年人群分成营养不良组、有营养不良风险组以及营养状况正常组,使用BIA判断他们是否患有肌肉减少症。将营养不良组老年人分为干预组和对照组,干预组予以HMB90天进行营养干预。结果利用BIA检测出社区老年男性患者肌肉减少症患病率为22.6%,老年女性为12.1%,四肢肌肉组织和身高平方的比值:营养状况正常组(男性为7.6±0.62,女性为6.23±0.15)高于营养不良风险组(男性为7.15±0.83,女性为5.84±0.26)及营养不良组(男性为6.42±0.54,女性为5.46±0.68),差异有统计学意义。营养不良组经过基柔(含HMB2.6g/d)干预90天后,干预组四肢肌肉组织较对照组增加,差异有统计学意义。结论营养不良的老年人更容易患肌肉减少症,HMB补充剂可以增加老年人肌肉含量,有预防肌肉减少的作用。Objective To evaluate the nutritional status of Shanghai Xinjing community old people with MNA-SF,to analyze the relationship between nutritional status and sarcopenia,and to observe the effect of nutritional intervention.Methods300old people aged65or older were randomly selected from the community and to were divided into three groups based on the scores of MNA-SF:normal nutritional status group,risk of malnutrition group and malnutrition group.BIA was used to determine whether patients were with sarcopenia in all three groups.Malnutrition group was divided into interventional group and control group,interventional group implemented HMB for90days nutritional intervention.Results The prevalence rate of sarcopenia in male and female communitydwelling elders was22.6%and12.1%by using BIA respectively.The appendicular skeletal muscle to height squared ratio in normal nutritional status group(7.6±0.62in males and6.23±0.15in females)was higher than those at risk of malnutrition group(7.15±0.83in males and5.84±0.26in females)and the malnutrition group(6.42±0.54in males and5.46±0.68in females).All differences were statistically significant(P<0.05).After intake of JiRou(HMB supplementation was2.6g/d)intervention for90days in malnutrition people,appendicular skeletal muscle in intervention group was statistically increased than control group(P<0.05).Conclusions People with malnutrition were more likely to suffered from sarcopenia.HMB supplementation had shown its increase effect on appendicular skeletal muscle and could attenuated muscle wasting in elderly people.
