咽鼓管球囊扩张联合鼓膜置管对于难治性分泌性中耳炎的疗效观察

摘　　要：目的探讨咽鼓管球囊扩张联合鼓膜置管对于难治性分泌性中耳炎的临床疗效。方法 50例难治性分泌性中耳炎患者,随机分为对照组和观察组,各25例。对照组单用鼓膜切开置管,观察组采用咽鼓管球囊扩张联合鼓膜置管,比较两组患者的治疗效果、症状改善情况。结果观察组治疗有效率为88%,显著高于对照组的64%,差异有统计学意义(P<0.05)。观察组治疗后言语频率平均听阈、咽鼓管功能障碍问卷(ETDQ-7)评分、耳峰压值、静态声顺值均明显优于对照组,差异有统计学意义(P<0.05)。结论咽鼓管球囊扩张联合鼓膜置管对于难治性分泌性中耳炎疗效显著,能有效改善听力,减少复发,值得在临床推广使用。Objective To explore the clinical effect of eustachian tube balloon dilation combined with tympanostomy tube for refractory secretory otitis media.Methods A total of50refractory secretory otitis media patients were randomly divided into control group and observation group,with25cases in each group.The control group received myringotomy with grommet insertion only,and the observation group received eustachian tube balloon dilation combined with tympanostomy tube.Comparison were made on treatment effect and symptom improvement in two groups.Results The observation group had significantly higher total treatment effective rate as88%than64%in the control group,and the difference had statistical significance(P<0.05).The observation group had clearly better word frequency pure-tone-average,eustachian tube dysfunction questionnaire(ETDQ-7)score,ear peak pressure value,static sound value after treatment than the control group,and the difference had statistical significance(P<0.05).Conclusion Combination of eustachian tube balloon dilation and tympanostomy tube shows significant curative effect for refractory secretory otitis media,which can effectively improve hearing and reduce recurrence.So it is worth clinical promotion and use.
