超声心动图筛查胎儿先天性心脏病的应用价值

摘　　要：目的:探讨应用彩色多普勒超声心动图多切面三节段法产前筛查胎儿先天性心脏病(CHD)的诊断价值。方法:回顾性分析5 000例应用彩色多普勒超声心动图产前筛查20~40周胎儿心脏,并通过尸体解剖和分娩后超声复查证实。结果:95例经产前超声诊断CHD,另有4例室间隔缺损和1例肺动脉狭窄漏诊,共100例CHD患儿均经随访和尸体解剖证实,CHD检出率1.9%(95/5 000)。结论:心血管畸形在胎儿中的发病率并不低,超声心动图是产前筛查CHD的有效方法;对于有高危因素的孕妇,应该早期进行超声心动图筛查,及时诊断,以减少CHD患儿的出生,对于优生优育十分重要。Objective:To explore the diagnostic value of color doppler echocardiography in multi section three section method in the prenatal screening of fetal congenital heart disease(CHD).Methods:5 000 cases of 20 to 40 weeks fetal heart with prenatal screening by color doppler echocardiography were retrospective analysis.They were confirmed by autopsy and ultrasonography after delivery.Results:95 cases were diagnosed CHD by prenatal ultrasound,and another 4 cases of ventricular septal defect and 1 case of pulmonary stenosis were missed diagnosis.100 cases of CHD were confirmed by follow up and autopsy.The detection rate of CHD was 1.9% (95/5 000).Conclusion:The incidence of cardiovascular malformations in the fetus is not low,and echocardiography is an effective method for prenatal screening of CHD.For pregnant women with high risk factors,it should be early echocardiography screening,timely diagnosis in order to reduce the birth of CHD patients.It is very important for eugenics.
