2型糖尿病及其并发症住院药费分析

摘　　要：目的:了解我院2型糖尿病及其并发症患者的住院药费情况,并分析影响住院药费的相关因素。方法:调查2010年1月-2014年12月我院住院部病案系统7271例2型糖尿病患者的住院药费,采用SPSS 21.0软件进行一般数据挖掘和统计作图,采用多元线性回归分析其影响因素。结果:2型糖尿病患者次住院药费中位数为2903.19元;住院药费逐年递增;住院药费中西药费占主导,中药费中以中成药费为主,中草药费为辅,中西药费构成比例大致为4∶6;2型糖尿病患者住院药费排名第一的科室为内分泌科;2型糖尿病足患者的住院药费用最高;住院药费随并发症种数的增加而增加;性别、住院天数、糖尿病神经并发症、糖尿病伴眼并发症和糖尿病肾病是2型糖尿病患者住院药费的主要影响因素。结论:2型糖尿病患者住院药费较高,西药费负担重,中医药价格优势不突出,各种慢性并发症增加了住院药费。Objective:To analyze the hospitalized drug cost for type2diabetic patients in our hospital and the influencing factors.Methods:Hospitalized drug cost for7271patients with type2diabetes from January2006to December2014in our hospital inpatient medical record system was investigated;general data mining and statistical mapping were established by SPSS21.0software;and the influencing factors was analyzed by multivariate linear regression.Results:The median of total hospitalized drug cost for type2diabetic patients was2903.19Yuan.The hospitalized drug cost increased year by year.In hospitalized drug cost,the cost of Western medicine is dominant,Chinese patent medicine fee plays an important role in TCM medicine cost,while TCM medicine fee is supplementary,and the ratio ofTCM medicine fee and Western medicine fee is approximately4∶6.The hospitalized drug cost for type2diabetes inpatients in the endocrinologydepartment ranked first.Type2diabetic foot inpatients cost the most.The hospitalized drug cost increased with the increasingnumber of complications.Gender,duration of hospitalization,diabetic neuropathy complications,diabetic eye complications and diabeticnephropathy are the main factors affecting the hospitalized drug cost for patients with type2diabetes mellitus.Conclusion:The hospitalized drug cost of type2patients is high.The burden of western medicine cost is heavy.Price advantage of TCM medicine is not prominent,and any kind of chronic diabetic complication increases the cost.
