吴成专运用正骨手法结合PFNA治疗老年股骨粗隆间骨折经验

摘　　要：目的:观察福建省基层老中医药专家吴成专运用传统中医正骨手法结合PFNA(股骨近端防旋髓内钉)内固定治疗老年股骨粗隆间骨折的疗效。方法:选取2014年12月-2015年12月福建中医药大学附属泉州市正骨医院下肢一科符合纳入标准的老年股骨粗隆间骨折患者75例,按治疗方法将其分为治疗组与对照组,其中治疗组36例,对照组39例,吴成专对治疗组在上牵引床前运用正骨手法对骨折端进行整复,然后再上牵引床牵引维持复位,并结合PFNA内固定,而对对照组采用直接上牵引床进行牵引复位结合PFNA内固定,对两组手术资料、辅助复位率、术后并发症、患肢下地部分负重时间、骨折愈合时间和髋关节功能评分等临床资料进行随访分析。结果:治疗组手术时间、术中出血量及辅助复位率较对照组明显减少,两组对比具有显著性差异(P<0.01);两组患肢下地部分负重时间及骨折愈合时间比较,差异无统计学意义(P>0.05);两组的并发症及优良率比较,差异无统计学意义(P>0.05)。结论:吴成专运用正骨手法结合PFNA内固定治疗老年股骨粗隆间骨折不仅疗效确切,而且更微创、更安全,可供广大骨科同道参考应用。Objective:To observe clinical effects of TCM bone setting manipulation plus proximal femoral nail anti-rotation on intertrochanteric fracture of the elderly in Fujian Province.Methods:75elderly patients were divided into the treatment group and control group,36cases in the treatment group and39cases in the control group.The treatment group was given TCM bone setting manipulation plus traction and PFNA.The control group took traction plus PFNA.Results:Compared with the control group,the operation time,the amount of blood loss and the reduction in the treatment group were significantly lower;there was a significant difference between the groups(P<0.01).The difference of limb partial weight-bearing time and fracture healing time in groups was not significant(P>0.05).There was no significant difference between groups in the complications and the excellent(P>0.05).Conclusion:Bone setting manipulation plusPFNA on intertrochanteric fractures in the elderly was not only effective,but also more minimally invasive and safer,could provide more reference for the majority of orthopedic.
