摘　　要：目的:探讨腰椎退变性侧弯并椎管狭窄症不同手术疗效及影像学结果。方法:收治腰椎退变性侧弯伴椎管狭窄症手术者35例,依据手术方式不同分为A组(单纯减压术式)6例、B组(减压固定术式)29例,分别通过影像学测量其矢状面及冠状面参数,对比分析各组的术后效果。结果:B组术后Cobb角、L_4/L_5倾斜度均显著低于术前,矫正成效差异具有统计学意义(P<0.05)。B组腰椎前凸角(LL)及骶骨倾斜角(SS)指标较术前改善(P<0.05)。结论:腰椎退变性侧弯伴椎管狭窄症处于平衡状态者可采取单纯椎管减压手术治疗。对于脊柱失平衡患者,改善冠状面及矢状面平衡,才能获得良好结果。Objective:To investigate the efficacy of different operation mode and imaging results of lumbar degenerative scoliosiswith spinal stenosis.Methods:35patients with lumbar degenerative scoliosis combined with spinal stenosis treated by surgery wereselected.They were divided into the group A(simple decompression type)in6cases and the group B(decompression fixation)in29cases.The sagittal and coronal parameters were measured by imaging,and the results were compared and analyzed.Results:TheCobb angle and L4/L5gradient of the group B were significantly lower than those before operation(P<0.05).The corrective effectwas statistically significant.The lumbar lordosis angle(LL)and sacral tilt angle(SS)indicators in the group B improved comparedwith those before operation(P<0.05).Conclusion:Paitents with degenerative lumbar spinal stenosis with scoliosis in equilibriumcan take simple vertebral canal decompression surgery.For patients with spinal imbalance,improve the coronal and sagittalbalance,in order to get good treatment results.
