尿液视黄醇结合蛋白在肾功能早期损伤中的诊断价值

摘　　要：目的对尿液视黄醇结合蛋白(RBP)在肾功能早期损伤诊断中的诊断作用进行分析。方法 300例肾功能损伤患者,根据测得的微量白蛋白/肌肝比值(mAlb/Cr)将其分为正常组(152例)、微量组(78例)和大量组(70例)。另取100例肾功能正常者为对照组。对各组患者RBP和蛋白含量的关系进行分析。结果微量白蛋白(mAlb)在尿液中的含量越多,其余蛋白的含量就越多,呈现出正相关,大量组>微量组>正常组和对照组。正常组患者尿液中的mAlb、α_1-微球蛋白(α_1-MG)和RBP的含量均高于对照组,差异具有统计学意义(P<0.05),其余三个指标两组比较差异无统计学意义(P>0.05);微量组的N-乙酰-β-D氨基葡萄糖苷酶(NAG)与正常组比较差异无统计学意义(P>0.05),其余指标均高于正常组,差异具有统计学意义(P<0.05)。大量组各指标均高于微量组,差异具有统计学意义(P<0.05)。经过相关性分析可知,RBP和尿液mAlb、免疫球蛋白(Ig)、转铁蛋白(Tr)、α_1-MG、NAG均呈正相关(r=0.903、0.706、0.700、0.391、0.302,P<0.05);正常组中RBP和mAlb、α_1-MG、NAG呈正相关(r=0.745、0.177、0.221,P<0.05)。微量组中的RBP和mAlb、Ig、NAG呈正相关(r=0.746、0.223、0.266,P<0.05)。大量组中RBP和α_1-MG呈正相关(r=0.372,P<0.05)。结论将尿液RBP作为肾功能早期损伤的诊断指标,准确率高,效果显著,实用性强,值得在临床上进一步推广使用。Objective To analyze diagnostic effect by retinol binding protein(RBP)in diagnosis of early renal impairment.Methods A total of300patients with renal impairment were divided by their microalbumin/creatinine(mAlb/Cr)into normal group(152cases),micro amount group(78cases)and large amount group(70cases).There were100healthy people with normal renal function as control group.Analysis was made on relationship between RBP and protein contents in each group.Results High content of urine microalbumin(mAlb)led to high content of other protein,and they were positively correlated as large amount group>micro amountgroup>normal group and control group.The normal group had higher urine mAlb,α1-microglobulin(α1-MG)and RBP contents than the control group,and their difference had statistical significance(P<0.05),and theirdifferences of the other three indexes had no statistical significance(P>0.05).There was no statistically significantdifference of N-acetyl-β-D-glucosaminidase(NAG)between the micro amount group and the normal group(P>0.05),while the micro amount group had other indexes all higher than the normal group,and their difference had statistical significance(P<0.05).The large amount group had all higher indexes than the micro amount group,and the difference had statistical significance(P<0.05).Correlation analysis showed positive correlation betweenRBP and urine mAlb,immune globulin(Ig),transferring(Tr),α1-MG,NAG(r=0.903,0.706,0.700,0.391,0.302,P<0.05).RBP in the normal group was positively correlated with mAlb,α1-MG and NAG(r=0.745,0.177,0.221,P<0.05).RBP in the micro amount group was positively correlated with mAlb,Ig and NAG(r=0.746,0.223,0.266,P<0.05).RBP in the large amount group was positively correlated withα1-MG(r=0.372,P<0.05).Conclusion As the diagnostic index for early renal impairment,urine RBP shows high accuracy,excellent effect and high practicability.It is worth further clinical promotion and application.
