翼状胬肉切除联合不同移植术式后干眼症的临床观察

摘　　要：目的研究翼状胬肉切除联合不同移植术式后干眼症的临床效果。方法 160例翼状胬肉患者,随机分为移植术A、B组,每组80例。移植术A组采用翼状胬肉切除联合自体结膜瓣移植术;移植术B组采用翼状胬肉切除联合自体角膜缘干细胞结膜瓣移植术。对比两组患者术前和术后1个月泪膜破裂时间、泪液分泌实验情况和翼状胬肉治疗效果、术后干眼症发生情况。结果术前两组患者泪膜破裂时间、泪液分泌实验情况比较差异无统计学有意义(P>0.05)。两组患者术后1个月泪膜破裂时间、泪液分泌实验情况和术前比较差异有统计学有意义(P<0.05)。移植术B组翼状胬肉治疗总有效率(95.00%)明显高于移植术A组(85.00%)(P<0.05)。移植术B组术后干眼症发生率(2.5%)明显低于移植术A组(12.5%)(P<0.05)。结论翼状胬肉切除联合联合自体角膜缘干细胞结膜瓣移植术的临床效果优于联合自体结膜瓣移植术效果,对泪液功能无不良影响,可降低术后干眼症发生率,值得推广。Objective To research clinical effect of xerophthalmia after pterygium excision combined with different transplantation measures.Methods A total of160patients with pterygium were randomly divided into transplantation group A and transplantation group B,with80cases in each group.The transplantation group A received pterygium excision combined with autogenous conjunctival flap transplantation,and the transplantation group B received pterygium excision combined with autogenous limbal stem cells and conjunctival flap transplantation.Comparison was made on tear film breakup time,lacrimal secretion experiment condition and curative effect for pterygium between the two groups before and in1month after operation,along with their occurrence of postoperative xerophthalmia.Results There was no statistically significant difference of tear film breakup time and lacrimal secretion experiment condition between the two groups before operation(P>0.05).The difference of tear film breakup time and lacrimal secretion experiment condition between the two groupshad statistical significance before and in1month after operation(P<0.05).The transplantation group B hadobviously higher total effective rate in pterygium treatment(95.00%)than the transplantation group A(85.00%)(P<0.05).The transplantation group B had much lower incidence of postoperative xerophthalmia(2.5%)than thetransplantation group A(12.5%)(P<0.05).Conclusion Combination of pterygium excision and autogenous limbal stem cells and conjunctival flap transplantation shows better clinical effect than combination of pterygium excision and autogenous conjunctival flap transplantation.This method shows no adverse affect on tear function,and it can lower incidence of postoperative xerophthalmia.It is worth promoting.
