右美托咪定在老年人坐骨神经联合股神经阻滞后的临床观察

摘　　要：目的观察右美托咪定在老年人坐骨神经联合股神经阻滞后的镇痛效果和不良反应。方法 60例坐骨神经联合股神经阻滞行膝关节手术的老年患者,按照随机数字表法分为对照组和观察组,各30例。对照组采用罗哌卡因注射液,观察组采用右美托咪定注射液,观察并记录两组患者术后4、8、24、36、48 h的视觉模拟评分法(VAS)评分及不良反应发生情况。结果观察组术后4、8、24、36、48 h的VAS评分均低于同时间的对照组,差异具有统计学意义(P<0.05)。对照组不良反应发生率为40.00%,观察组不良反应发生率为16.67%,观察组不良反应发生率明显低于对照组,差异具有统计学意义(P<0.05)。结论右美托咪定在老年人坐骨神经联合股神经阻滞中具有较好的镇痛效果,能有效减少不良反应的发生,临床应用价值高。Objective To observe analgesic effect and adverse reactions of dexmedetomidine after senile sciatic nerve combined with femoral nerve block.Methods A total of60senile patients receiving knee surgery under sciatic nerve combined with femoral nerve block were divided by random number table into control group and observation group,with30cases in each group.The control group received ropivacaine injection,and the observation group received dexmedetomidine injection.Observation and records were made on visual analogue scale(VAS)scores and occurrence of adverse reactions in postoperative4,8,24,36,48h in the two groups.Results The observation group had all lower VAS scores in postoperative4,8,24,36,48h than those in thecontrol group,and the difference had statistical significance(P<0.05).The control group had incidence of adverse reactions as40.00%,and the observation group had incidence of adverse reactions as16.67%.The observationgroup had obviously lower incidence of adverse reactions than the control group,and the difference had statisticalsignificance(P<0.05).Conclusion Dexmedetomidine shows excellent analgesic effect in senile sciatic nerve combined with femoral nerve block,and it can effectively reduce adverse reactions,along with high clinical application value.
