慢性肾衰继发性甲旁亢患者血清Klotho蛋白水平及影响因素分析

摘　　要：目的分析慢性肾衰继发性甲旁亢(SHPT)患者血清Klotho蛋白水平及其影响因素。方法选取行维持性血液透析的SHPT患者50例为SHPT组,健康体检者10例为对照组,分别检测血肌酐(Cr)、尿素氮(BUN)、碱性磷酸酶(ALP)、血钙、血磷、甲状旁腺激素(PTH)水平,酶联免疫吸附法测定血清成纤维细胞生长因子-23(FGF-23)、Klotho蛋白水平。结果 (1)SHPT组患者血Klotho蛋白水平明显低于对照组(P<0.05),血Cr、BUN、ALP、血钙、血磷、PTH、FGF-23水平明显高于对照组(P<0.05);(2)SHPT组患者血Klotho蛋白与血Cr、BUN、ALP、血钙、血磷、PTH、FGF-23均呈负相关(P<0.05);多元逐步回归分析显示,只有FGF-23、Cr、BUN进入方程,是Klotho水平的影响因素。结论 SHPT患者血Klotho蛋白水平明显降低,FGF-23是影响Klotho蛋白水平的重要因素。Objective To analyse the level and influential factors of Klotho protein in secondary hyperparathyroidism(SHPT)resulted from chronic renal failure.Methods50SHPT patients and10heslthy people were selected.The serum creatinine(Cr),blood urea nitrogen(BUN),alkaline phosphatase(ALP),calcium,phosphorus,parathyroid hormone(PTH)were detected.The serum fibroblast growth factor-23(FGF-23)and Klothoprotein were measured by enzyme linked immunosorbent assay technology.Results(1)The serum Klotho was significantly lower in SHPT group than in control group(P<0.05);the serum Cr,BUN,ALP,calcium,phosphorus,PTH,FGF-23were significantly higher in SHPT group than in controlgroup(P<0.05).(2)In SHPT group,there were negative correlation between Klotho and Cr,BUN,ALP,calcium,phosphorus,PTH,FGF-23(P<0.05),Multiple stepwise regression analysis showed that the influence factors of Klotho were FGF-23,Cr,BUN.Conclusion The serum Klotho in SHPT patients was significantly decreased,and FGF-23was an important influence factor of Klotho.
