摘 要:目的:探索检查体位优化技术对减少颈部CT带状伪影的应用价值。方法:选取30例因临床需要复查颈部CT患者,使用自身对照试验来对比优化体位与传统体位的差异。优化体位,即肩部外展上提法;传统体位,即标准摆位方法。对优化体位和传统体位的图像伪影进行三分制盲法评分,即严重伪影、中等伪影及轻度或无伪影,并使用秩和检验对评分进行比较。测量图像噪声值,记录检查的容积CT剂量指数(CTDIvol),使用配对t检验对两种体位的辐射剂量和图像噪声进行评估。结果:优化体位的评分为(2.94±0.243)分,显著高于传统体位的评分(2.03±0.3)分,差异有统计学意义(Z=-5.396,P<0.001);优化体位的图像噪声(7.91±1.17)明显低于传统体位图像噪声(10.62±2.20),差异有统计学意义(t=6.682,P<0.01);优化体位和传统体位的CTDI_(vol)平均值分别为(8.80±1.29)mGy和(8.77±1.24)m Gy,差异无统计学意义(t=0.304,P=0.763)。结论:颈部CT检查中,通过外展上提肩部的优化体位可减少下颈部的带状伪影,明显提高图像质量。Objective:To explore the value of optimized technique of positioning strategy for minimizing streak artifact in neck computed tomography(CT)for patient.Methods:30outpatients that need follow up neck CT examinations were enrolled the study.Selfcontrol study was adopted to compare the two positioning methods(optimized positioning:pushed patients'shoulders outward and cephalad stretching;traditional positioning:standard positioning without particular optimization).The images artifacts were evaluated by blind trisection scale(severe,moderate,minimum or no artifact).Wilcoxon rank sum test was used to compare the scores of the optimized positioning and the traditional positioning.The image noise was detected and the volume CT dose index(CTDIvol)was recorded,and paired t test was used to compare and evaluate the differences for image noise and CTDIvol,respectively,between the two positioning methods.Results:The score of optimized positioning(2.94±0.24points)was significantly higher than that of the traditional positioning score(2.03±0.3points)(Z=-5.396,P<0.001).The image noise of optimized positioning(7.91±1.17)was significantly lower than that of the traditional positioning(10.62±2.20)(t=6.682,P<0.01).The average CTDIvol of optimized positioning and traditional positioning were(8.80±1.29)mGy and(8.77±1.24)mGy,respectively,and there was no significant difference between the two methods(t=0.304,P=0.763).Conclusion:The optimized positioning strategy for patient can minimize the streak artifacts and improve image quality in the neck CT.
