低分子肝素联合激素、环磷酰胺治疗难治性肾病综合征疗效观察

摘　　要：目的探讨低分子肝素联合激素、环磷酰胺治疗难治性肾病综合征的疗效。方法 60例难治性肾病综合征患者作为研究对象,随机分为观察组和对照组,各30例。对照组采取激素联合环磷酰胺治疗,观察组采取激素、环磷酰胺、低分子肝素联合治疗,观察两组治疗效果。结果治疗前,两组患者的尿素氮、肌酐、24 h尿蛋白定量水平比较,差异无统计学意义(P<0.05);治疗后,观察组患者的尿素氮(7.02±0.55)mmol/L、肌酐(80.08±5.94)μmol/L、24 h尿蛋白定量(1.02±0.39)g水平均显著低于对照组(8.34±0.63)mmol/L、(86.55±6.88)μmol/L、(1.51±0.32)g,差异具有统计学意义(P<0.05)。结论对难治性肾病综合症患者采取激素、环磷酰胺、低分子肝素联合治疗,能显著降低尿素氮、肌酐、24 h尿蛋白定量等指标水平,改善患者的预后质量。Objective To investigate curative effect by low molecular heparin combined with hormone and cyclophosphamide in the treatment of refractory nephropathy syndrome.Methods A total of60patients with refractory nephropathy syndrome as study subjects were randomly divided into observation group and control group,with30cases in each group.The control group received hormone combined with cyclophosphamide for treatment,and the observation group received combined treatment by hormone,cyclophosphamide and low molecular heparin.Curative effects of the two groups were observed.Results Before treatment,there was no statistically significant difference of urea nitrogen,creatinine,and24h urine protein quantitation levels between the two groups(P<0.05).After treatment,the observation group had urea nitrogen as(7.02±0.55)mmol/L,creatinine as(80.08±5.94)μmol/L,and24h urine protein quantitation as(1.02±0.39)g,which were all much lower than(8.34±0.63)mmol/L,(86.55±6.88)μmol/L and(1.51±0.32)g in the control group,and their difference had statistical significance(P<0.05).Conclusion Implement of combined treatment by hormone,cyclophosphamide and low molecular heparin for patients with refractory nephropathy syndrome can remarkably lower urea nitrogen,creatinine,and24h urine protein quantitation levels,and improve quality of prognosis in patients.
