血液透析治疗危重型肾综合征出血热60例临床疗效分析

摘　　要：目的探讨血液透析治疗危重型肾综合征出血热的临床疗效。方法 60例危重型肾综合征出血热患者,均行血液透析治疗。采用随机数字表法分为对照组和观察组,各30例。对照组患者实施匀速超滤治疗,观察组患者实施阶梯超滤治疗,比较两组患者的临床疗效。结果与治疗前比较,治疗后两组患者尿素氮、肌酐、乳酸脱氢酶、谷丙转氨酶降低,总蛋白、白蛋白增加,差异均有统计学意义(P<0.05)。治疗后观察组患者尿素氮、肌酐、乳酸脱氢酶、谷丙转氨酶低于对照组,总蛋白、白蛋白高于对照组,差异均有统计学意义(P<0.05)。观察组患者尿蛋白消失时间、肌酐达标时间、尿素氮达标时间早于对照组,差异有统计学意义(P<0.05)。观察组患者不良事件发生率3.3%低于对照组的20.0%,差异有统计学意义(P<0.05)。结论匀速超滤、阶梯超滤均可用于危重型肾综合征出血热的临床治疗,其中阶梯超滤的效果更好,值得临床推广使用。Objective To explore the clinical curative effect of hemodialysis in the treatment of severehemorrhagic fever with renal syndrome.Methods A total of60patients with severe hemorrhagic fever withrenal syndrome all receiving hemodialysis were divided by random number table method into control group andobservation group,with30cases in each group.The control group received ultrafiltration treatment at a constantspeed,and the observation group received ladder ultrafiltration treatment.Clinical curative effect was comparedin two groups.Results Comparing with before treatment,both groups had lower urea nitrogen,creatinine,lactatedehydrogenase and alanine aminotransferase and higher total protein and albumin after treatment,and theirdifference had statistical significance(P<0.05).After treatment,the observation group had lower urea nitrogen,creatinine,lactate dehydrogenase and alanine aminotransferase than the control group and higher total protein andalbumin in the control group.Their difference had statistical significance(P<0.05).The observation group hadearlier urine protein disappearance time,creatinine time requirds and urea nitrogen time requirds than the controlgroup,and their difference had statistical significance(P<0.05).The observation group had lower incidence ofadverse events as3.3%than20.0%in the control group,and the difference had statistical significance(P<0.05).Conclusion Both constant and ladder ultrafiltration can be applied in clinical treatment of severe hemorrhagicfever with renal syndrome,and ladder ultrafiltration shows better effect.So it is worth clinical promotion and use.
