摘 要:目的探讨慢性乙型肝炎患者的肾功能早期损害以及影响因素。方法 182例慢性乙型肝炎患者作为病例组,采用慢性肾脏病流行病合作工作组(CKD-EPI)方程计算其肾小球滤过率(eGFR),根据C-eGFR结果分为有早期肾损害患者与无肾损害患者。选取同期84例健康体检者作为对照组,检测病例组与对照组肝肾功能。结果病例组丙氨酸转氨酶(ALT)和血肌酐(Scr)明显高于对照组,而血清白蛋白(Alb)、血尿素氮(BUN)明显低于对照组,差异有统计学意义(P<0.05);慢性乙肝早期肾损害患者年龄为(47.30±6.29)岁高于无肾损害的(38.25±8.11)岁,慢性乙肝早期肾损害Alb为(38.03±5.62)g/L低于无肾损害者的(41.29±8.01)g/L,差异有统计学意义(P<0.05);慢性乙肝有早期肾损害和无肾损害患者性别、体质量指数(BMI)、ALT、Scr和BUN比较,差异无统计学意义(P>0.05);多元线性回归分析结果显示年龄和Alb是慢性乙肝患者C-eGFR的影响因素。结论慢性乙型肝炎患者可发生肾功能早期损害,而年龄和Alb是影响肾功能的影响因素,临床应注意对患者肾功能进行监测。Objective To explore early renal function damage in chronic hepatitis B patients and itsimpact factors.Methods A total of182chronic hepatitis B patients as case group received chronic kidney diseaseepidemiology collaboration(CKD-EPI)to calculate their estimated glomerular filtration rate(eGFR).They weredivided by C-eGFR outcomes into patients with early renal damage and patients without non renal damage.Therewere84concurrent healthy people in physical examination as control group.Hepatorenal function in the case group and the control group were detected.Results The case group had obviously higher alanine aminotransferase(ALT)and serum creatinine(Scr)than the control group and obviously lower serum albumin(Alb)blood urea nitrogen(BUN)than the control group.Their difference had statistical significance(P<0.05).Chronic hepatitis B patients with earlyrenal damage had higher age as(47.30±6.29)years old than(38.25±8.11)years old in non renal damage patients,Chronic hepatitis B patients had lower Alb as(38.03±5.62)g/L than(41.29±8.01)g/L in non renal damagepatients.Their difference had statistical significance(P<0.05).Chronic hepatitis B patients with or without earlyrenal damage had no statistically significant difference in body mass index(BMI),ALT,Scr and BUN(P>0.05).Multiple linear regression analysis resutls showed that age and Alb are impact factors of C-eGFR in chronichepatitis B patients.Conclusion Chronic hepatitis B patients mayhave early renal function damage,and age andAlb are impact factors of renal function.So more attention should be payed to moitor renal function in clinic.
