五瓣法在轻度杯状耳矫正术中的应用

摘　　要：目的介绍五瓣法在轻度杯状耳畸形中的应用并总结其优点。方法从2012年至今,临床收治的10例(12侧)改进Tanzer的分型中的A、B型患者,男4例,年龄8~21岁,平均年龄(11±3.69)岁。单侧8例,双侧2例,临床表现为耳廓上1/3轻度卷曲,手法复位后耳廓软骨组织结构清晰,高低较健侧大小相当,术中以耳轮卷曲下缘皮肤为主轴,长度为卷曲皮肤张力带,设计五瓣法,紧贴软骨膜面掀起皮瓣,耳软骨舒展自然,交错对位缝合皮瓣,并调整前后缘皮瓣的松紧辅助矫正软骨长期下垂的内聚应力。结果耳廓下垂校正,手术切口一期愈合,无皮瓣坏死,术后3月、6月、1年、2年随访,术区瘢痕不明显,耳廓发育无受限,外形好。结论五瓣法在单纯皮肤软组织环缩形成的杯状耳畸形的治疗中有效可取。Objective This paper introduces the application of five flaps in mild cup ear deformity in and summarize its advantages.Methods Since2012,10patients(12sides)type of improved Tanzer in A,B patients,4were male,aged8-21years old,the average age was(11±3.69)years.8cases were unilateral.2cases of bilateral clinical manifestations were on the auricle1/3slightly curly,after manual reduction of cartilage tissue structure is clear,the level of a healthy size,intraoperative helix curls the skin lower edge of the spindle,the length of curly skin tension belt,five flap,close to the cartilage membrane surface off the ear cartilage flap,natural stretch staggered,sutured the skin flap,and adjust the tightness of trailing edge flap assisted correction of cartilage long drooping the cohesive stress.Results The auricle ptosis correction,incision healed without skin flap necrosis after operation in March,June,1years,two years of follow-up,postoperative scar is not obvious,the development of auricle not restricted,good appearance node.Conclusion Five valve method is effective in the treatment of skin and soft tissue ring constriction of the cup ear deformity.Department of plastic and reconstructive surgery,Shanxi hospital,Shanxi Academy of Medical Sciences,China
