摘　　要：目的探讨鼻内镜下鼻内开窗术治疗上颌骨囊肿手术径路及手术方法的可行性及临床效果。方法对15例上颌骨囊肿患者行鼻内镜下鼻内开窗术。根据囊肿与上颌窦及鼻腔的位置关系,选择在鼻底、下鼻道或泪前隐窝等处开窗,手术切除开窗处囊肿壁,必要时使囊肿与上颌窦融合为一个腔,保证囊肿腔与上颌窦、鼻腔引流通畅。结果 15例患者均一次手术成功,未出现并发症。术后随访6~24个月,囊肿术腔均于术后2或3个月上皮化,囊肿腔引流通畅,囊肿无复发。结论鼻内镜下鼻内开窗术治疗上颌骨囊肿简便、创伤小、恢复快、安全有效,且复发率低。Objective To explore the feasibility and evaluate its efficacy of transnasal marsupializationof maxillary cyst under nasal endoscope.Methods15cases of maxillary cyst were treated by endoscopicmarsupialization in nasal.According to the situation of maxillary cysts,the fenestration of bottom nasal was openedin6cases,the fenestration of inferior nasal meatus was opened in7cases and inferior nasaI meatus was openedthrough the prelacrimal duct recess in2cases under the nasal endoscope.With partial removal of the cyst wall,thecyst and maxillary sinus was fused into a cavity if necessary.This ensured nasal drainage through the cyst cavityand nasal cavity or maxillary sinus.Results The operations of the15patients were success without complications.All patients were followed up for6to24months after operation.Operation cavity to complete epithelization in2to3months,the cyst cavity drained well with no recurrence.Conclusion Endoscopic marsupialization in nasal is afeasible alternative for management maxillary cyst.It makes the procedure simple,less traumatic,quick recovery,definite curative effect and low recurrence rate.
